Résumé |
La réanimation d’un patient en état de mort encéphalique (EME) a pour finalité la réalisation d’un prélèvement d’organes et de tissus. La qualité de la réanimation influence directement le nombre d’organes prélevés. La mort encéphalique se définit comme la perte irréversible des fonctions encéphaliques, en particulier celles du tronc cérébral. Le passage en EME est associé à une cascade d’événements hémodynamiques, hormonaux, inflammatoires, variables dans leur expression et leur intensité, mais susceptibles d’altérer le fonctionnement des organes. L’index bispectral (BIS) et le Doppler transcranien constituent 2 aides au diagnostic d’EME. L’absence de ventilation spontanée doit légalement être attestée par un test d’apnée, qui est souvent seul garant de la mort du tronc cérébral. Le respect des objectifs de réanimation garantit la qualité optimale des greffons. La Coordination Hospitalière de Prélèvement d’Organes et de Tissus est associée à la prise en charge du patient dès le diagnostic clinique d’EME. Le respect des bonnes pratiques de l’entretien avec les proches réduit leur opposition au prélèvement d’organes. Une réanimation d’attente permet le passage en EME et le prélèvement d’organes de patients d’emblée inaccessibles à un traitement curatif.

The resuscitation of a patient after brain death (BD) aims at organ and tissue procurement. During this period, the quality of the patient management is correlated with the number of withdrawn organs. BD is defined by the irreversible loss of the functions of the whole brain, including brain stem. BD occurrence is associated with haemodynamic, hormonal and inflammatory changes that may be variable in severity but account for organ dysfunction. Bispectral index and transcranial Doppler are useful tools for BD diagnosis. In France, the disappearance of spontaneous breathing must be assessed by the apnoea test, which confirms the death of the brain stem. Donor management goals should improves the viability of the organs before withdrawal, targeting standardised critical care end points. Organ procurement organisation should be early involved in the donor management since the beginning of the diagnosis. When Good Practices Recommendations are respected at the time the relatives are approached for organ donation, consent is more frequently obtained. Patients with brain lesions without therapeutic options may be admitted in Intensive Care Units to be managed in the expectation of BD occurrence in view of organ procurement.
