Contexte |
Le syndrome de désadaptation psychomotrice (SDPM), qui associe anxiété, phobie de la chute, troubles psychomoteurs et posturaux, est une complication grave des chutes ou des immobilisations prolongées. La thérapie d’exposition à la réalité virtuelle (TERV) a montré son efficacité pour le traitement des troubles anxieux et phobiques des sujets âgés mais n’a pas été étudiée spécifiquement dans le SPDM.
Objectif |
Il s’agit d’une étude pilote pour évaluer l’acceptabilité et la tolérance de la TERV dans le SDPM du sujet âgé.
Méthodes |
Trois séances d’exposition à la réalité virtuelle (RV) simulant une exposition progressive au vide antérieur ont été proposées à 10 patients (9 femmes et 1 homme ; 86,8±4,56 ans) souffrant de SDPM. L’acceptabilité a été déduite du fait de souhaiter participer aux séances suivantes. La tolérance a été évaluée par l’éventuelle augmentation de la pression artérielle (PA) et de la fréquence cardiaque (FC), avant, pendant et après l’exposition à la RV, et par la comparaison des scores à l’échelle d’anxiété « State Trait Anxiety Inventory forme Y-A » (STAI) avant-après.
Résultats |
Aucun sujet n’a refusé de poursuivre les séances. Aucune élévation des PA et FC n’a été détectée aux différents temps de mesure. Les valeurs du STAI avant et après exposition chaque jour n’ont pas varié significativement pour les premiers (t=1,43 ; p=0,185), deuxièmes (t=0,54 ; p=0,601) et troisièmes jours (t=−1,18 ; p=0,269), mais 20 % des patients ont quand même rapporté un sentiment d’angoisse. La motricité et l’équilibre se sont améliorés (évaluation par le « Test Moteur Minimum » et l’échelle « Backward Disequilibrium Scale »).
Conclusion |
La TERV semble être bien tolérée et acceptée pour la rééducation du SPDM chez des patients âgés.

Background |
The Psychomotor Disadaptation Syndrome (PDS) is one of the most damaging fall-related complications in the elderly. This syndrome comprises anxiety, fear of falling, psychomotor and postural disorders, and eventually leads to a loss of autonomy. Virtual Reality Exposure Therapy (VRET) has been found effective in the treatment of anxiety disorders, but has not been studied as a treatment for PDS.
Objective |
This pilot study evaluated the acceptability and tolerance of VRET among elderly subjects with PDS.
Methods |
Three VRET sessions were offered to hospitalized elderly patients with PDS. The patients selected were to be without serious cognitive impairment, or vestibular or cerebellar syndromes. Acceptability was evaluated by the patient's agreement to participate in further sessions after the first. Tolerance was evaluated on the basis of any increases in blood pressure (BP) and heart rate (HR) before, during and after the exposures sessions, and of scores on the State-Trait Anxiety Inventory Y-A (STAI) (scores out of 80, the higher the score the more serious is the anxiety) administered before and after the sessions. BP and HR values were compared using the T-Test, and STAI scores using ANOVA. P<0.05 was considered significant.
Results |
All patients who were asked to participate in the study agreed: 10 patients (9 women and 1 man; 86.8±4.56 years). BP and HR did not increase during the sessions. The STAI scores before and after exposure did not significantly vary across the first (t=1.43; P=0.185), second (t=0.54; P=0.601) and third day, whether before or after exposure (t=−1.18; P=0.269).
Conclusion |
VRET seems to exhibit good acceptability and tolerance in the rehabilitation of PDS among hospitalized elderly people. A larger study also assessing VRET efficacy could be conducted to back up these results.
