L’infection d’une prothèse articulaire est peu fréquente (1 à 2 %) mais grave, justifiant des stratégies adaptées pour en réduire l’incidence. L’anesthésiste-réanimateur a un rôle important à jouer dans cette prise en charge. Lors de la consultation d’anesthésie, il doit détecter les facteurs de risque spécifique (diabète déséquilibré, anémie, traitements immunomodulateurs, traitement anticoagulant…) et entreprendre des actions correctrices pour en minorer l’impact. En périopératoire immédiat, l’adaptation des antibiotiques et du traitement anticoagulant est aussi une étape fondamentale de la prévention de l’infection postopératoire dont on distingue deux types : l’infection de la cicatrice opératoire (superficielle) et l’infection ostéo-prothétique (plus profonde). La prise en charge d’une infection de prothèse est une urgence. L’antibiothérapie initiale probabiliste basée selon les recommandations de la HAS sur l’association vancomycine-tazocilline doit être adaptée rapidement aux résultats bactériologiques mais aussi à la fonction rénale du patient. La mise en place d’un abord veineux adapté à un traitement antibiotique prolongé et veinotoxique (PICCline, Midline, cathéter central) doit être envisagée dès la première reprise chirurgicale.

Infection of a joint prosthesis is a rare (1–2%) but serious event, justifying appropriate strategies to reduce its incidence. The anesthesiologist plays an important role in this management. During the anaesthetic consultation, risk factors (diabetes mellitus, anaemia, immunosuppressive or anticoagulant treatments…) must be identified and corrected to reduce their impact. In the perioperative period, adaptation of antibiotics and anticoagulants is also a fundamental step in preventing postoperative infection. Two types of complications have been identified: infection of the operative scar and osteo-prosthetic infection (deeper). The management of a prosthesis infection is an emergency. The initial probabilistic antibiotherapy based on the HAS recommendations on the vancomycin-tazocillin association must be rapidly adapted to the bacteriological results but also to the patient's renal function. Long duration placement of an intravenous line (PICCline, Midline, catheters) for prolonged antibiotics administration should be considered as soon as the first surgical revision.
