Résumé |
Introduction |
La bronchite plastique (BP) est caractérisée par la formation des moules épais ramifiés et fortement adhérents, qui obstruent partiellement ou complètement la lumière trachéobronchique. Elle constitue une complication évolutive d’affections respiratoires et/ou cardiaques préexistantes ou idiopathique qui se révèlent habituellement pendant l’enfance.
Observation |
Nous rapportons l’observation d’une femme, âgée de 48 ans, qui était admise pour exacerbation de son asthme. L’imagerie thoracique a objectivé la présence d’opacités parenchymateuses diffuses. L’exploration endoscopique de l’arbre trachéobronchique a montré l’obstruction étendue de l’arbre bronchique gauche par des moules bronchiques. L’évolution était favorable après l’ablation des moules intra bronchiques et le traitement par corticothérapie orale.
Conclusion |
La BP est une affection rare, généralement secondaire, une maladie respiratoire ou cardiaque chronique sous-jacente. Bien qu’habituellement de révélation précoce pendant l’enfance, la BP peut être diagnostiquée à l’âge adulte. Le traitement repose sur l’extraction des moules par voie endoscopique et sur le traitement de la pathologie sous jacente.

Introduction |
Plastic bronchitis (PB) is characterized by the formation of thick branched and strongly adherent molds, which partially or completely obstruct the brachial lumen. It constitutes an evolutionary complication of preexisting or idiopathic respiratory and/or cardiac affections which usually appear during childhood.
Observation |
We report the case of a 48-year-old woman, who was admitted for exacerbation of her asthma. Chest imaging has shown the presence of diffuse parenchymal opacities. Endoscopic exploration of the tracheobronchial tree showed extensive obstruction of the left bronchial tree by bronchial molds. The evolution was favorable after removal of the intrabronchial molds and treatment with oral corticosteroid therapy.
Conclusion |
BP is a rare affection, usually secondary to an underlying chronic respiratory or heart disease. Although usually an early onset in childhood, BP can be diagnosed in adulthood. The treatment is based on the extraction of mules using the endoscope and on the treatment of the underlying pathology.
