Flow cytometric assessment of vasodilator-stimulated phosphoprotein: Prognostic value of recurrent cardiovascular events after acute coronary syndromes - 05/12/08

Background |
Clopidogrel fails to elicit an adequate antiplatelet response in 4–30% of patients. Assessing the phosphorylation of intraplatelet vasodilator-stimulated phosphoprotein (VASP) is an easy and reliable method of evaluating biological response to clopidogrel.
Aim |
To assess the prognostic value of clopidogrel in patients with an acute coronary syndrome (ACS) without persistent ST-segment elevation.
Methods |
We studied clopidogrel response prospectively in 49 patients treated with a loading dose of 300mg clopidogrel followed by a maintenance dose of 75mg/day. VASP index was calculated from the median fluorescence intensity (MFI) of samples incubated with prostaglandin E1 (PGE1) and adenosine diphosphate according to the formula [(MFI(PGE1)–MFI(PGE1–ADP))/MFI(PGE1)]×100, and was determined at baseline and at days1 and 4 after starting clopidogrel. We correlated VASP index with occurrence of recurrent cardiovascular events over six-month follow-up.
Results |
There was a significant stepwise decrease in VASP index from baseline (86±6%) to day1 (71±13%) and day4 (61±16%; p<0.001) with marked interindividual variability. Patients who experienced recurrent cardiovascular events displayed a higher VASP index compared with those free of events (76±3% versus 59±16%, p=0.006). Five of six recurrent events occurred in patients in the upper quartile of VASP index measured at day4. The best cut-off of platelet reactivity index of VASP to predict high-risk ACS patients was at 70%.
Conclusion |
Assessment of VASP index in ACS patients identifies low responders to clopidogrel who are at increased risk of recurrent cardiovascular events.

Justification |
Le clopidogrel ne permet pas d’obtenir une réponse antiplaquettaire adéquate chez 4 à 30 % des patients. L’évaluation de la phosphorylation de la protéine intraplaquettaire vasodilator-stimulated phosphoprotein (VASP) est une méthode simple et fiable pour évaluer la réponse biologique au clopidogrel.
Objectifs |
Évaluer la valeur pronostique de VASP dans les syndromes coronariens aigus (SCA) sans sus décalage persistant du segment ST.
Méthodes |
Nous avons étudié prospectivement 49patients après une dose de charge de 300mg de clopidogrel suivi d’une dose d’entretien de 75mg par jour. L’index de VASP était calculé à partir de l’intensité de fluorescence moyenne (IFM) des échantillons incubés avec de la prostaglandine E1 (PGE1) et de l’adénosine diphosphate (ADP), selon la formule [(IFM(PGE1)–IFM(PGE1–ADP))/IFM(PGE1)]×100 et était déterminé avant clopidogrel, après un jour et quatre jours de clopidogrel. Nous avons corrélé l’index de VASP avec la survenue de récidive de syndrome coronarien aigu après un suivi de six mois.
Résultats |
Une diminution progressive de l’index de VASP était notée de (86±6 %) à j0 à (71±13 %) à j1 puis (61±16 % ; p<0,001) à j4 avec une importante variabilité interindividuelle. Les patients qui ont présenté dans le suivi un nouvel événement cardiovasculaire avaient un index de VASP plus élevé par rapport à ceux qui n’ont pas eu d’événement (76±3 % versus 59±16 % ; p=0,006). Cinq des six événements sont survenus chez les patients du quartile supérieur de l’index de VASP mesuré à j4. Le meilleur seuil d’index de VASP pour prédire les patients à haut risque d’événements cardiovasculaire après un SCA était de 70 %.
Conclusion |
L’évaluation de l’index de VASP dans les SCA identifie des faibles répondeurs au clopidogrel qui sont à risqué élevé d’événements cardiovasculaires.
