Resumen |
La notion de traitement chondroprotecteur ou structuromodulateur a été définie afin d'isoler une classe thérapeutique capable de prévenir, de stabiliser ou de réparer des lésions arthrosiques. La méthode de référence actuellement utilisée pour l'évaluation d'un effet structuromodulateur dans la gonarthrose et la coxarthrose est la mesure du pincement de l'interligne articulaire sur des radiographies standard répétées. Cette mesure peut se faire manuellement ou de manière assistée par ordinateur. Plusieurs techniques radiologiques ont été décrites pour mesurer la hauteur de l'interligne au compartiment fémorotibial interne de genou et supéro-externe de hanche. La majorité des études de structuromodulation ont porté sur les molécules dites « anti-arthrosiques d'action lente », actuellement prescrites pour leur effet symptomatique retardé dans l'arthrose. Si la plupart de ces traitements ont montré des effets in vitro intéressants, aucun d'entre eux n'a à ce jour prouvé un effet structuromodulateur cliniquement pertinent.

Abstract |
The terms “chondroprotective” and “structure-modifying” were coined to identify a class of drugs capable of preventing, stabilizing, or repairing joint damage caused by osteoarthritis. The method of reference for evaluating structure-modifying effects in knee or hip osteoarthritis is measurement of joint space loss on serial plain radiographs. Joint space width can be measured manually or by a computer. Several radiological techniques have been described for measuring joint space width in the medial femorotibial compartment of the knee and the superolateral part of the hip. Most studies of potentially structure-modifying effects evaluated slow-acting drugs for osteoarthritis, which are currently used for their delayed symptomatic effect in osteoarthritis. Although most of these agents have shown promising effects in vitro, none has been proven to produce clinically meaningful structure-modifying effects in humans with osteoarthritis.
