Fongémie fatale à Trichosporon asahii : à propos des deux premiers cas à Sfax (Tunisie) - 08/06/10

Résumé |
Les Trichosporon sont des levures de plus en plus incriminées dans la survenue d’infections profondes, notamment chez les patients d’oncohématologie. Nous rapportons deux observations de septicémie à Trichosporon asahii diagnostiquées au laboratoire de parasitologie et mycologie du CHU Habib-Bourguiba de Sfax, Tunisie. Notre premier patient présentait une leucémie aiguë myéloïde avec une fièvre persistante malgré la mise en place d’une antibiothérapie à large spectre. Le second cas avait une anémie avec une fièvre résistante aux antibiotiques. T. asahii a été isolé des hémocultures dans les deux cas. Les patients ont été mis sous amphotéricine B mais l’évolution était fatale. Les trichosporonoses sont rares mais particulièrement graves chez les sujets immunodéprimés avec un pronostic souvent péjoratif malgré le traitement antifongique.

Trischosporon species are emerging opportunistic yeasts in neutropenic patients with underlying hematological malignancies. We report two observations of invasive trichosporonosis diagnosed at the Parasitology and Mycology Laboratory of Sfax University Hospital (South of Tunisia). The first case had an acute myeloid leukemia and a persistent fever despite the use of broad-spectrum antibiotics. The second case had anaemia with resistant fever to antibiotics. Trichosporon asahii was isolated from blood cultures in the two cases. Antifungal therapy with amphotericin B was initiated but the evolution was fatal. Trichosporonosis is a rare but potentially highly aggressive infection with a bad prognosis among immunosuppressed patients despite antifungal therapy.
