Les manifestations articulaires du VIH sont-elles des spondylarthropathies ? - 01/01/03

Resumen |
Au Congo-Brazzaville, la séroprévalence du VIH est de 7 à 8 %, le sida représente la première cause des arthrites aseptiques (60 %).
Patients, matériel et méthode. - Les critères ESSG et d'Amor de spondylarthropathies ont été étudiés chez 83 patients infectés par le virus de l'immunodéficience humaine et hospitalisés en 8 ans dans le service de rhumatologie du CHU de Brazzaville pour « arthrite liée au VIH ».
Résultats. - Quatre-vingt-trois patients VIH+ tous au stade IV CDC présentent une polyarthrite : 66 cas (80 %) ou une oligo-arthrite : 17 cas (20 %). Un seul patient répond aux critères ESSG et d'Amor de spondylarthropathie avec 6 points, 1 patient totalise 5 points et 15 ne dépassent pas 3 points. Les arthrites non érosives, asymétriques, lentement régressives en 4-8 semaines sous anti-inflammatoires non stéroïdiens, siègent le plus souvent aux membres inférieurs : le genou (84 %), la cheville (59 %), le gros orteil (23 %). Aux membres supérieurs, le poignet (41 %), le coude (29 %), les MCP et IPP (25 %) représentent les principales localisations.
Conclusion. - Ces arthrites ne répondent pas aux critères ESSG ou d'Amor de spondylarthropathies.

Abstract |
In Congo-Brazzaville, the seroprevalence of HIV-infection is in the 7-8% range, and AIDS is the leading cause of aseptic arthritis (60% of cases).
Patients, material and methods. - The ESSG and Amor's criteria for spondylo-arthropathy were evaluated in 83 patients with HIV-infection admitted for aseptic arthritis to the rheumatology department of the Brazzaville teaching hospital, over an 8-years period.
Results. - All 83 patients were CDC stage IV; 66 (80%) had polyarthritis and 17 (20%) oligo-arthritis. A single patient met ESSG and Amor's criteria, with 6 points; 1 patient had 5 points and 15 had 3 points. The joint involvement was asymmetrical and nonerosive. Nonsteroidal anti-inflammatory therapy ensured resolution of the manifestations within 4 to 8 weeks. The most common sites of involvement were the knees (84%), ankles (59%), and great toes (23%) at the lower limbs and the wrists (41%), elbows (29%), and metacarpophalangeal and interphalangeal joints (25%) at the upper limbs.
Conclusion. - Inflammatory arthritis in HIV patients does not meet ESSG or Amor's criteria for spondylo-arthropathy.
