Aspergillose primitive de la fosse cérébrale postérieure chez un patient immunocompétent - 11/03/11

Résumé |
Les aspergilloses cérébrales surviennent dans la grande majorité des cas au cours d’une aspergillose invasive, avec dissémination hématogène à partir de lésions pulmonaires. La localisation cérébrale, non rare, est un des pires critères pronostiques au cours de l’aspergillose invasive. Nous rapportons le cas d’un patient hospitalisé en service de neurochirurgie pour hémiparésie associée à un syndrome cérébelleux dans un contexte apyrétique. L’exploration radiologique objective une collection au niveau de la fosse cérébrale postérieure. Une biopsie stéréotaxique est réalisée. Elle ramène des fragments biopsiques et du pus. L’analyse microbiologique et anatomopathologique permet l’isolement et l’identification d’Aspergillus fumigatus. L’originalité de cette observation est liée à la localisation rare au niveau de la fosse cérébrale postérieure et au fait que le patient est immunocompétent et qu’aucune localisation primaire n’est retrouvée. Le patient est par ailleurs porteur d’une hépatite virale B de découverte fortuite lors de son hospitalisation. Il est mis sous traitement par l’amphotéricine B. L’évolution est marquée par une méningite post-dérivation. Le patient est mis sous traitement antibiotique puis opéré pour exérèse en masse de la tumeur. Il décède suite à un coma osmolaire associé à une thrombopénie et une insuffisance rénale secondaire à son traitement par l’amphotéricine B.

Cerebral aspergillosis arises in the great majority of cases during an invasive aspergillosis with hematogene scattering from the lung hurts. The cerebral, not rare location is one of the worse criteria forecast during the invasive aspergillosis. We report the case of patient who was hospitalized in the neurosurgery department for syndrome of increased intracranial pressure, hemiparesis and cerebellar syndrome in febrile context. The radiological exploration objectified a collection of the posterior fossa. A stereotactic biopsy was performed. It collected fragments biopsy and pus. The pathological and microbiological analysis allowed the identification of Aspergillus fumigatus. The originality of this observation comes from the rare location in the posterior fossa of aspergillosis and because the patient is immunocompetent and no primary location is found. The patient presents however a viral hepatitis B of fortuitous discovery. He is put under treatment by amphotericin B. The evolution is marked by meningitis comment-diversion. Antibiotics are prescribed, and then the patient is operated for total ablation of the tumor. He dies following an osmolar coma associated with thrombopenia and a secondary renal insufficiency due to his treatment by the amphotericin B.
