La prise en charge chirurgicale des cataractes infantiles a bénéficié des progrès réalisés chez l’adulte. Elle reste toutefois spécifique à l’enfant et s’intègre dans une stratégie thérapeutique globale adaptée à chaque cas. Elle doit donc être guidée par une équipe ophtalmopédiatrique habituée à ce type de prise en charge. Nous développons ici, en fonction du type de cataracte, uni- ou bilatérale, opacifiée précocement ou progressivement, le choix du moment de l’intervention, le choix initial de stratégie thérapeutique, les explications aux parents, et le bilan préopératoire. La technique opératoire « standard » actuelle de phacoaspiration en technique bimanuelle avec capsulorhexis postérieur et vitrectomie antérieure est décrite point par point. Les indications de l’implantation, le choix du type d’implant et de sa puissance selon l’âge sont détaillés, que ce soit pour une implantation initiale ou secondaire.

Management of congenital or infantile cataracts benefited from progresses realized in adult surgery. It stays however specific to children’s eyes and is incorporated in a global therapeutic strategy. It must be guided by a pediatric ophthalmogical team used to this kind of treatment. We report here, function of the type of cataract, uni or bilateral, congenital or infantile, the choice of the timing of surgery, of the therapeutic strategy, parental information, and preoperative evaluation. The actual standard of surgery is bimanual phacoaspiration with posterior capsulorhexis and anterior vitrectomy. It is described step by step. Indication of lens implantation, choice of the type and power of IOL function of age of the child is described, be it for initial or secondary lens implantation.
