Incontinence anale : étude échographique et manométrique - 01/01/01

But de l'étude : Le but de cette étude était d'évaluer les conséquences fonctionnelles et cliniques des défects échographiques du sphincter anal parmi les patients ayant une incontinence anale.
Patients et méthode : De janvier 1997 à avril 2001, 100 patients consécutifs (90 femmes et dix hommes) ont été adressés pour bilan d'incontinence anale. La gravité clinique de l'incontinence anale a été évaluée par le calcul du score de Wexner coté de 0 à 20. Une endosonographie anale et une manométrie anale vectorielle ont été réalisées.
Résultats : L'endosonographie anale était normale chez 42 patients. Chez 58 patients, il existait un défect : 26 défects isolés (trois du sphincter interne, 23 du sphincter externe), 32 défects combinés (sphincter externe et interne). Il existait une relation significative entre la gravité clinique de l'incontinence et l'importance du défect du sphincter externe exprimée en degrés. L'amplitude de la contraction volontaire était significativement diminuée dans le groupe avec défect combiné par rapport au groupe sans défect. L'index d'asymétrie (repos et contraction volontaire) était augmenté dans le groupe avec défect combiné par rapport au groupe sans défect et par rapport au groupe avec défect isolé.
Conclusion : Les lésions échographiques du sphincter anal sont fréquentes en cas d'incontinence anale. Elles ont un retentissement clinique et fonctionnel significatif.

Study aim: The goal of this study was to evaluate the clinical and functional consequences of anal sphincter echographic lesions in patients with fecal incontinence.
Patients and method: From January 1997 to April 2001, 100 patients with fecal incontinence (90 women, 10 men) were prospectively explored by transanal ultrasound and vectormanometry. Internal and external sphincter defects, as well as their extension, were analyzed with regard to their clinical and functional expressions (resting and squeeze anal sphincter pressures, anal sphincter asymetry index).
Results: Among the 100 patients, 42 had no echographic lesions, 58 had a defect : 26 had an isolated defect (internal sphincter (n=3), external sphincter (n=23) and 32 had combined internal and external sphincter defects. Squeeze anal pressures were significantly lower in the group of patients with combined defects, compared to the group with isolated defect or without defect. Anal asymetry index was higher in the group of patients with combined defects, compared to the other groups. There was a positive correlation between the radial extension of the defect and the importance of clinical complaints.
Conclusion: Echographic anal sphincter lesions are frequent in fecal incontinence and are clinically and functionally significant.
