Aqueous humor contamination during phacoemulsification in a university teaching hospital - 18/03/12

To evaluate the intraoperative contamination of the anterior chamber in eyes undergoing phacoemulsification in a university teaching hospital.
Design |
Prospective, non-randomized clinical trial.
Methods |
This study included 113 eyes of 113 consecutive patients undergoing cataract surgery performed by experienced surgeons and residents. Phacoemulsification was conducted through a scleral tunnel incision or through a corneal incision. The intraocular lens was implanted with an injector. Aqueous fluid was obtained at the end of surgery before viscoelastic removal, with a cannula through the corneal paracentesis. Povidone-iodine 10% solution was used to prepare the eyebrow and eyelids and povidone-iodine 5% to disinfect the ocular surface. All patients were given a single oral dose of 400mg ofloxacin 2h before surgery. No preoperative antibiotics were administered locally.
Results |
Anterior chamber fluid aspirates were positive for bacteria in two eyes (1.8%). No eye developed endophthalmitis during the follow-up period.
Discussion and conclusion |
Low anterior chamber bacterial contamination rates may be achieved under routine phacoemulsification in a teaching hospital practicing careful antisepsis measures.

Introduction |
La plupart des cas d’endophtalmie postopératoire résulte de la pénétration intraoculaire de germes durant l’intervention. Le but de cette étude est d’évaluer le taux de contamination bactérienne de la chambre antérieure après phacoémulsification dans un service hospitalo-universitaire comprenant des chirurgiens en formation.
Patients et méthodes |
Cent treize yeux de 113 patients consécutifs opérés de la cataracte en 2008 furent inclus dans cette étude prospective, non randomisée. La chirurgie fut effectuée par phacoémulsification par des chirurgiens confirmés ou en apprentissage. L’implant intraoculaire fut inséré grâce à un injecteur. L’humeur aqueuse fut prélevée en fin d’intervention avant l’ablation du viscoélastique. La préparation cutanée préopératoire du patient fut effectuée à l’aide de povidone iodée 10 % suivie d’une irrigation de la surface oculaire avec de la povidone iodée 5 %. Les patients reçurent une dose orale de 400mg d’ofloxacine deux heures avant la chirurgie. Aucune antibioprophylaxie topique préopératoire ne fut associée.
Résultats |
Aucun cas d’endophtalmie postopératoire ne survînt. Seuls deux prélèvements d’humeur aqueuse furent positifs, soit un taux de contamination bactérienne de la chambre antérieure de 1,8 %.
Discussion |
Un taux minime de contamination bactérienne de la chambre antérieure après phacoémulsification fut observé dans notre structure. Des mesures d’antisepsie et d’asepsie soigneuses permettent en partie d’expliquer ces résultats.
