Douleur chronique post-thoracotomie révélant une large hernie pulmonaire : prise en charge chirurgicale curative 32 ans plus tard - 09/06/13

Résumé |
Un patient de 54ans a consulté pour une douleur thoracique gauche chronique évoluant depuis deux ans. Sa profession consistait à soulever de lourdes charges dans un marché. Son histoire médicale est marquée par une notion de thoracotomie postérolatérale gauche en 1979. L’interrogatoire a retrouvé une douleur localisée autour de la thoracotomie et en sous-mammaire, de type neurologique (brûlures, picotements, engourdissement et névralgies), et mécanique (étirement, sensation de pesanteur). Ce phénomène douloureux s’est aggravé progressivement, avec des accès paroxystiques lors d’un effort important depuis le début de la symptomatologie. Un traitement par carbamazépine prescrite par paliers jusqu’à 600mg/j a permis une amélioration clinique. L’examen clinique a retrouvé une voussure importante surtout après manœuvres de Valsalva. La radiographie thoracique a objectivé une perte de substance osseuse avec image de clarté située en dehors de la paroi thoracique. Le diagnostic de hernie pulmonaire large a été évident. L’intervention a consisté en une réduction du sac herniaire, puis renforcement de la paroi thoracique par un treillis non résorbable de polytéréphtalate d’éthylène (type Mersuture*). Les suites postopératoires ont été simples. Le patient est sorti de l’hôpital après une semaine. La radiographie thoracique de contrôle a été satisfaisante. Le traitement de sortie de l’hôpital a été sous forme de paracétamol 1g trois fois par jour, et systématiquement de carbamazépine à la dose de 600mg/j avec diminution progressive jusqu’à son arrêt après quatre mois. Après 21 mois de recul, le patient est asymptomatique, avec une bonne rigidité de la paroi thoracique.

A 54-year-old man consulted for left chest chronic pain evolving for 2years. His job consisted in lifting heavy load in a market. His medical history found a notion of left posterolateral thoracotomy in 1979. Interrogation found a pain localized around the thoracotomy and inframammary, neurological in form (burning, tingling, numbness, and neuralgia), and mechanical (stretching, feeling of heaviness). This pain gradually worsened, with paroxysms after a major effort from the onset of symptoms. Treatment with carbamazepine prescribed increments up to 600mg per day resulted in a clinical improvement. Clinical examination found a large arch mainly after Valsalva maneuvers. The chest radiograph objectified loss of bone with clarity image outside of the chest wall. The diagnosis of wide pulmonary hernia was obvious. The intervention consisted of a reduction of the hernia sac, and strengthening the chest wall by a non-absorbable mesh polyethylene terephthalate (type Mersuture*). The postoperative course was uneventful. The patient was discharged from hospital after a week. Chest X-ray control was satisfactory. Treatment of discharge was as paracetamol 1g three times a day, and always as carbamazepine at a dose of 600mg per day with gradual decrease until it stops after 4months. After 21months of decline, the patient is asymptomatic, with good rigidity of the chest wall.
