Riassunto

L'infection par le virus de l'immunodéficience humaine reste un problème majeur à l'échelon planétaire et ses manifestations ophtalmologiques sont toujours d'actualité, plus particulièrement dans les régions du monde ne disposant pas d'un accès aisé aux traitements antirétroviraux modernes. Toutes les parties de l'œil et des annexes oculaires peuvent être atteintes d'affections inflammatoires, infectieuses ou néoplasiques. Parmi elles, les infections opportunistes du segment postérieur sont sans conteste les plus dévastatrices au plan visuel, avec un pronostic souvent très sombre si elles ne sont pas traitées, incluant essentiellement les rétinopathies herpétiques nécrosantes, la rétinite à cytomégalovirus, la rétinochoroïdite toxoplasmique et la syphilis oculaire. Certaines entités nosologiques ont fait leur apparition depuis l'avènement des thérapies antirétrovirales, principalement dans le contexte de la reconstitution immune. Celle le plus communément décrite est l'uvéite de reconstitution immune rencontrée chez des individus ayant développé une rétinite à cytomégalovirus et bénéficiant d'un traitement antirétroviral efficace. Le traitement et la prise en charge des différentes atteintes ophtalmologiques rencontrées dans le contexte de l'infection par le virus de l'immunodéficience humaine sont souvent complexes, et requièrent incontestablement une approche multidisciplinaire incluant l'ophtalmologiste, l'interniste et l'infectiologiste. Bien que l'incidence d'infections opportunistes oculaires - principalement la rétinite à cytomégalovirus - soit en net recul dans les régions du monde disposant d'un accès relativement aisé aux traitements antirétroviraux, l'identification précoce de ces infections, la connaissance de leurs manifestations cliniques, ainsi que l'instauration sans délai d'un traitement adéquat sont essentielles pour minimiser la morbidité visuelle de ces affections, à plus forte raison considérant l'accroissement substantiel des flux migratoires en provenance de régions géographiques dans lesquelles la prévalence et l'incidence de l'infection par le virus de l'immunodéficience humaine restent un problème majeur de santé publique.

Abstract

Infection with human immunodeficiency virus is still a major problem at the global level and its ophthalmologic manifestations are still relevant, particularly in regions of the world that do not have easy access to modern antiretroviral treatments. All parts of the eye and ocular adnexa may be involved in inflammatory, infectious or neoplastic damages. Among them, opportunistic infections of the posterior segment are unquestionably the most devastating on the visual side, with often very gloomy prognosis if they are not treated, mainly including herpetic necrotizing retinopathies, cytomegalovirus retinitis, toxoplasmic retinochoroiditis and ocular syphilis. Some nosologic entities have emerged since the advent of antiretroviral therapies, primarily in the context of immune reconstitution. The most commonly described one is the immune recovery uveitis encountered in individuals who developed cytomegalovirus retinitis and receiving an effective antiretroviral treatment. Treatment and management of various ophthalmic conditions encountered in cases of human immunodeficiency virus infection are often complex and undoubtedly require a multidisciplinary approach which includes the ophthalmologist, internist and infectiologist. Although the incidence of opportunistic ocular infections - mainly cytomegalovirus retinitis - is generally declining in regions of the world with a relatively easy access to antiretroviral treatment, early identification of these infections, knowledge of their clinical manifestations as well as the establishment without delay of proper treatment are essential to minimize visual morbidity of these diseases, even more so considering the substantial increase in migration from geographical areas in which the prevalence and incidence of human immunodeficiency virus infection remains a major public health problem.
