Résumé

Le traumatisme du segment antérieur de l'oeil reste fréquent dans notre environnement et touche préférentiellement le sujet masculin et l'enfant. Les accidents de travail sont pourvoyeurs de traumatismes qui entraînent de graves conséquences socioprofessionnelles et médicolégales chez ces hommes jeunes. Les accidents domestiques augmentent en nombre et leur prévention n'est pas encore organisée. L'uniformisation de la terminologie s'impose en matière de description des traumatismes et il semble nécessaire d'opter pour une classification unique : la Birmingham Eye Trauma Terminology a l'avantage d'être claire et adoptée par les sociétés savantes internationales. Les différentes lésions de cette classification sont étudiées cliniquement et leur prise en charge thérapeutique est détaillée. Les lésions angulaires des contusions entraînent des troubles tensionnels délétères sur le long terme. L'existence d'une plaie cristallinienne en particulier chez le jeune enfant est un facteur de pronostic fonctionnel défavorable. La présence de corps étranger lors de traumatisme à globe ouvert augmente le risque d'endophtalmie. Les moyens actuels d'imagerie oculaire (biomicroscopie ultrasonore, tomographie à cohérence optique, scanner, résonnance magnétique nucléaire) permettent d'affiner le diagnostic des lésions et d'adapter le geste chirurgical. Les progrès des techniques chirurgicales et en particulier des sutures microchirurgicales améliorent le pronostic du traumatisme du segment antérieur, mais les atteintes du segment postérieur sont malheureusement souvent associées. Pour éviter leur apparition, une prophylaxie drastique doit être imposée dans les milieux professionnels et la sensibilisation des parents s'avère indispensable en leur conseillant la mise à l'écart de tout objet tranchant de l'environnement de l'enfant. Les progrès de la prévention routière ont rendu exceptionnelles les plaies oculaires par bris de pare-brise, mais la présence des airbags a créé d'autres types de lésions oculaires.

Abstract

Traumatism of the anterior segment of the eye is frequent and occurs preferentially in men and children. Professional accidents are the cause of many traumas that induce severe social, professional and medico-legal consequences in affected subjects. Rates of domestic accidents are increasing but their prevention remains to be adequately organized. A standardized description must be used to describe mechanical eye injuries; the Birmingham Eye Trauma Terminology (BETT) provides an unambiguous, consistent and comprehensive tool, recognized and endorsed by several international societies. This classification distinguishes the closed globe (with contusion, lamellar laceration and superficial foreign body) from the open globe (with rupture, penetrating and perforating laceration, intraocular foreign body. The various types of anterior segment traumas are described clinically together with their therapeutic management. Irido-corneal angle injuries in closed eye traumas induce post-traumatic chronic glaucoma; prognostic factors for poor outcome in children include traumatic cataract; intraocular foreign body increases the risk of endophthalmitis. The different modalities currenly used to study anterior segment structures are able to determine anterior chamber angle depth, define the various mechanisms of glaucoma and find foreign bodies: Ultrasound BioMicroscopy, Optical Coherent Tomography and Magnetic Resonance Imaging are the most commonly used modalities to evaluate eyes after trauma and help for surgery. Progresses in the surgical treatment of anterior segment trauma and the improved quality of suture may ameliorate the outcome but the functional result depends on the associated posterior segment injuries. In workplaces, a very strict and efficient prophylaxis must be launched to prevent anterior segment trauma. More adequate adult supervision and educational measures are necessary to reduce the prevalence of ocular injuries in children. Ocular injuries related to traffic accidents have decreased owing to glass quality improvement but the airbags are responsible for news lesions.
