Riassunto

La douleur est présente chez de nombreux patients souffrant d'un syndrome d'hyperlaxité, le plus fréquent d'entre-eux étant le syndrome d'hypermobilité articulaire bénigne (SHAB). Ces patients peuvent aussi avoir des problèmes psychosociaux, qui retentissent sur leur santé. Le principal symptôme reste la douleur articulaire, musculaire ou ligamentaire, renforçant les conséquences de microtraumatismes répétés ; d'autres facteurs sont également associés comme l'arthrose ou la fibromyalgie. Les facteurs neurophysiologiques pourraient se comporter ici comme des stimuli nociceptifs. La douleur et la souffrance d'origine viscérale peuvent résulter d'une laxité du tissu conjonctif thoracique, abdominal ou pelvien entraînant prolapsus utérin ou rectal, prolapsus de la valve mitrale, ou pneumothorax spontané. Chez des enfants, l'hyperlaxité articulaire est un facteur important et souvent méconnu des symptômes rhumatologiques, ignoré ou mis à tort sur le compte d'un rhumatisme inflammatoire. La prise en charge de la douleur et de la gêne des syndromes d'hyperlaxité requièrent connaissance, patience, compassion et compréhension. Souvent les résultats des traitements classiques (médicamenteux ou chirurgicaux), comme ceux que l'on utilise pour d'autres affections rhumatologiques, sont décevants ; des approches innovantes sont nécessaires. Parmi celles-ci, pour laquelle quelques résultats démontrés ont été obtenus, on peut citer la physiothérapie, la prescription d'orthèses, la rééducation proprioceptive, et des techniques plus récentes de prise en charge de la douleur comme les thérapies cognitives comportementales.

Abstract

Pain, distress and joint hyperlaxity. - Pain dominates the lives of many patients with hyperlaxity syndromes, most commonly the Benign Joint Hypermobility Syndrome (BJHS/EDS). As a result they may experience psychosocial problems, which in many cases severely affects their healthy functioning. Above all is the overriding chronic pain in joints, muscles and ligaments, which arise from an inherent predisposition to the effects of everyday trauma, but other factors such as associated osteoarthritis or fibromyalgia are also important. There may also be neurophysiological factors at play producing nociceptive enhancement. Pain and distress of visceral origin can result from laxity of connective tissue within or providing support for the abdominal, thoracic or pelvic viscera leading to hernia uterine and/or rectal prolapse, mitral valve prolapse or spontaneous pneumothorax. In children joint hyperlaxity is an important (and often unrecognised) source of rheumatic symptoms, which may be ignored or erroneously ascribed to juvenile idiopathic arthritis. The management of pain and distress in the hyperlaxity syndromes requires skill, patience, compassion and understanding. Often the results of conventional anti-rheumatic therapy (including anti-rheumatic drugs and surgery) as applied to other rheumatic diseases are disappointing and innovative approaches are required. Amongst these, for which evidence of efficacy is available are physiotherapeutic and orthotic stabilisation of hyperlax joints, proprioceptive enhancement and the newer pain management techniques including cognitive behavioural therapy.
