Résumé

L'analyse du fond d'oeil est un besoin constant de l'ophtalmologiste et peut nécessiter l'utilisation d'explorations complémentaires que sont les rétinographies, les clichés en autofluorescence, l'angiographie en fluorescence et l'angiographie infrarouge au vert d'indocyanine. Ces différents examens d'imagerie souvent complétés de tomographies en cohérence optique vont permettre d'affiner le diagnostic des pathologies du fond d'oeil et leur prise en charge. Ils sont réalisés progressivement en commençant par les examens non invasifs : rétinographies et clichés en autofluorescence. Les rétinographies sont déclinées avec différents filtres monochromatiques permettant chacun d'améliorer le contraste de certaines structures du fond d'oeil. Les clichés en autofluorescence réalisent une carte topographique de la distribution de la lipofuscine au sein de l'épithélium pigmentaire et d'autres fluorophores qui peuvent s'accumuler de façon pathologique dans la rétine externe et l'espace sous-rétinien. Les examens angiographiques sont ensuite réalisés si nécessaire apportant la composante dynamique de l'imagerie du fond d'oeil. L'angiographie en fluorescence permet d'analyser la circulation rétinienne et choroïdienne et contribue au diagnostic de nombreuses affections de la rétine, de la choroïde et du nerf optique. Depuis cinquante ans, les progrès dans l'interprétation des résultats de l'angiographie, après injection de fluorescéine, sont allés de pair avec les progrès dans la connaissance des maladies rétiniennes, à tel point que le diagnostic de nombreuses affections repose sur les résultats de cet examen. Outre le diagnostic, les résultats de l'angiographie à la fluorescéine permettent de prendre des décisions thérapeutiques, de guider les traitements, notamment la photocoagulation au laser de la rétine, et d'en apprécier les résultats. Les avancées technologiques avec la diffusion de l'informatisation et la numérisation des images ont permis encore d'étendre les applications de l'angiographie en fluorescence en pratique clinique quotidienne et dans le domaine de la recherche. Plus récente, l'angiographie infrarouge au vert d'indocyanine permet une analyse plus spécifique de la circulation choroïdienne et de ses relations avec la rétine et est devenue une technique de routine pour détecter les anomalies choroïdiennes.

Abstract

The ophthalmologist's need for fundus analysis is a constant and can require the use of complementary explorations such as retinography, autofluorescence images, fluorescein angiography, and indocyanine green infrared angiography. These different imaging exams, often completed with optical coherence tomography, can refine the diagnosis of fundus diseases and their management. They are performed progressively beginning with the noninvasive exams: retinographs and autofluorescence images. Retinographs are taken with different monochromatic filters, each of which can improve the contrast of certain structures of the fundus. Autofluorescence images are taken with a topographic map of the lipofuscin distribution of the pigmentary epithelium and other fluorophores that can accumulate pathologically in the external retina and the subretinal space. Angiographic exams are then performed if necessary, contributing the dynamic component of fundus imaging. Fluorescein angiography analyzes retinal and choroidal circulation and contributes the diagnosis of a number of retinal, choroidal, and optic nerve diseases. Over the past 50 years, progress in the interpretation of angiography results, after fluorescein injection, has equaled the advancing knowledge of retinal diseases, to such an extent that the diagnosis of many diseases is based on the results of this exam. Other than the diagnosis, the results of fluorescein angiography have made it possible to make therapeutic decisions, notably laser photocoagulation of the retina, and to assess the results. Technological advances, with the spread of computerization and image digitalization, have again extended the applications of fluorescein angiography in daily clinical practice and research. More recently, indocyanine green infrared angiography provides a more specific analysis of choroidal circulation and its relations with the retina, becoming a routine technique to detect choroidal anomalies.
