La maladie de Horton est une vascularite du sujet âgé qui affecte les artères de moyen et grand calibre, et qui prédomine dans le territoire céphalique. Les complications sont ischémiques et peuvent affecter l'œil, l'orbite ou les voies visuelles, la cécité étant la complication la plus redoutée. Cette cécité est le plus souvent due à une névrite optique ischémique antérieure aiguë. Ainsi, chez un patient de plus de 50ans, toute manifestation ischémique oculaire, transitoire ou permanente, doit faire rechercher une maladie de Horton par un interrogatoire ciblé et un bilan inflammatoire en urgence. Cependant, une symptomatologie clinique fruste ou l'absence de syndrome inflammatoire biologique ne doivent pas faire exclure le diagnostic. Seul l'examen histologique d'une biopsie d'artère temporale permet de poser le diagnostic de certitude en objectivant une artérite à cellules géantes. Le traitement repose sur la corticothérapie, qui doit être instaurée dès la suspicion du diagnostic. L'enjeu est de protéger l'œil ou les voies visuelles afin d'éviter la survenue d'une cécité irréversible, ou d'empêcher la maladie de se bilatéraliser. La corticothérapie ne constitue donc pas un traitement curatif des baisses d'acuité visuelle constituées, qui ont un pronostic presque toujours défavorable.

Giant cell arteritis is a vasculitis occuring in the elderly which affects arteries of medium and large diameter, and predominates in the cephalic territory. Complications are of ischemic type and can affect the eye, the orbit or the visual pathways, blindness being the most feared complication. This blindness is usually due to acute anterior ischemic optic neuropathy. Thus, in patients over 50years, any ocular ischemic events, transient or permanent, should be investigated for a giant cell arteritis by directed questioning and inflammation assessment in emergency. However, vague clinical symptoms or absence of inflammatory syndrome should not exclude the diagnosis. Only histological examination of a temporal artery biopsy will allow a positive diagnosis by objectifying a giant cell arteritis. Treatment consists of steroids, which should be initiated immediately on suspicion of the diagnosis. The challenge is to protect the eye or visual pathways to prevent the occurrence of irreversible blindness, or the bilateralisation of the disease. Steroids do not constitute a cure for visual impairment that are already installed, which almost always have an unfavorable prognosis.
