This is a case report of a patient with Lyme disease presenting with meningitis and a transient palsy of the right abducens nerve. T1-weighted post-contrast MRI showed marked enhancement of the right oculomotor, trigeminal and abducens nerves.
Four comparable cases are reported from the literature and though the finding is not pathognomonic of Lyme disease, enhancement of cranial nerves on post-contrast MRI may help in diagnosis.
