Réaction allergique aux antituberculeux majeurs - 15/10/09

Résumé |
L’allergie aux antituberculeux est relativement rare. Elle peut mettre en jeu le pronostic vital. Sa reconnaissance est indispensable, afin de pouvoir traiter efficacement la tuberculose. Pour ce patient âgé, bacillifère, les réactions sont survenues dès le troisième jour. Il présente un rash des membres inférieurs. La réintroduction séparée des antibiotiques, en milieu hospitalier, ne pose pas de problème pour l’isoniazide puis l’éthambutol. Lors de l’introduction de la rifampicine, il n’y a aucune réaction, tandis que l’adjonction du pyrazinamide s’accompagne de bronchospasme. La reprise du traitement sans pyrazinamide est suivie d’une réaction anaphylactique. Le bilan biologique était normal. Le diagnostic clinique et les tests cutanés ont mis en évidence une double réaction Rifampicine–pyrazinamide. Il n’y a pas eu de dosage d’IgE spécifiques. L’induction d’une immunotolérance à la rifampicine a permis un traitement par trithérapie (isoniazide, rifampicine, éthambutol). Lors du contrôle ophtalmique, la survenue d’une complication oculaire nous obligera à remplacer l’éthambutol par une fluoroquinolone. Le diagnostic de l’allergie n’est pas toujours aisé, les examens biologiques sont peu pertinents, de réalisation difficile en hôpital général. Plusieurs antibiotiques peuvent être concernés comme ce fut le cas.

Allergic reactions to antituberculosis drugs are rare. Sometimes they are fatal, so they must be diagnosed for an efficacy treatment of the active tuberculosis. In this case, allergic reactions occurred on the third day of antituberculosis treatment. He has a rash on his legs. Antibiotics were reintroduced separately in hospital, without problem for isoniazide and ethambutol. Apparently there is no problem with rifampicin, but pyrazinamide reintroduction gives a bronchospasm. After giving the three others drugs, he has an anaphylactic reaction. This patient has no biologic disorder. Clinical signs and skin tests permitted the diagnosis of allergy both to rifampicin and pyrazinamide. Induction of immune tolerance makes possible tuberculosis treatment with the three major drugs. Rapidly he develops an ophthalmic complication needed replacement of ethambutol by fluoroquinolone. Recognizing allergic reaction is sometimes difficult, biologic is not always positive and its realization sometimes difficult in general hospital. We can have several allergic drugs.
