Gangrène des doigts après injection accidentelle en intra-artériel de floxacilline : à propos d’un cas - 09/05/11

Résumé |
L’injection accidentelle intra-artérielle de certains médicaments est source d’une morbidité importante. Une vascularisation anormale, une situation difficile ou une erreur du personnel contribuent à cette situation. Nous présentons une observation récente d’une fille de sept ans, hospitalisée pour amputation traumatique de la troisième phalange de l’index gauche. L’enfant a été mise sous floxacilline injectable en postopératoire. Le troisième jour, après une injection accidentelle en intra-artériel, l’enfant a présenté une ischémie aiguë du membre supérieur droit. Une intervention urgente a mis en évidence une plaie intimale de l’artère humérale et une thrombose des axes radial et ulnaire. Une thrombectomie a été réalisée associée à une aponévrotomie de l’avant-bras et une héparinothérapie postopératoire a été démarrée. L’évolution a été marquée par une gangrène distale des cinq doigts de la main droite. À travers notre observation, et après une revue de la littérature, nous allons rappeler la physiopathologie et les possibilités thérapeutiques en cas d’injection accidentelle en intra-artériel de certains médicaments, tout en essayant de sensibiliser le personnel soignant sur ce risque.

Accidental intra-arterial injection of drugs is rare in pediatric patients. It is a source of considerable morbidity. Vessel proximity, aberrant vasculature, difficult procedures, and clerical errors can contribute to accidental intra-arterial injection. We report a case of a 7-year-old girl who had an accidental intra-arterial injection of 500mg of floxacillin in her right elbow for open wound of the index. Initial signs and symptoms consisted of intense forearm pain and cyanosis. Two to 3hours later, discoloration and nail bed pallor became evident, with no radial pulse. Rapid development of signs of acute ischemia prompted urgent brachial artery embolectomy and exploration and forearm fasciotomy. Postoperative heparin was administered. Postoperatively gangrene of the five fingers of the right hand developed. We present a comprehensive review of literature, highlighting available information on symptoms, pathogenesis, sequelae and management strategies for accidental intra-arterial injection. We believe that all physicians and ancillary personnel who administer intravenous therapy should be aware of this serious risk.
