Chez l’adulte, le « Post-Anesthetic Discharge Scoring System » (PADSS) a été construit pour garantir le retour à domicile après une chirurgie ambulatoire. L’objectif de ce travail était d’évaluer une adaptation pédiatrique de ce score : le Pediatric-PADSS (Ped-PADSS).

Méthodes |
Il s’agissait d’une étude prospective, observationnelle, monocentrique pour des patients ambulatoires. Le Ped-PADSS est construit sur 5 items chacun coté 0, 1 ou 2 : hémodynamique, état d’éveil, nausées/vomissements, douleur et saignement. Un résultat ≥9/10 validait la sortie si l’anesthésiste-réanimateur ne souhaitait pas revoir le patient, si les parents ne souhaitaient pas revoir l’anesthésiste-réanimateur ou s’il n’y avait ni dysphonie ni dyspnée. La sortie était validée par l’anesthésiste-réanimateur et le chirurgien. Le Ped-PADSS était réalisé à l’insu des équipes soignantes, une heure après le retour dans le service et répété toutes les heures. Outre les données démographiques, le recueil comportait les heures d’autorisation de sortie de l’anesthésiste-réanimateur, du chirurgien et du Ped-PADSS, la durée d’hospitalisation postopératoire.
Résultats |
Sur 150 patients, 148 étaient autorisés à sortir avec le Ped-PADSS et l’anesthésiste-réanimateur, un patient était sorti malgré un Ped-PADSS<9. Un patient était hospitalisé pour un saignement chirurgical en accord avec l’anesthésiste-réanimateur, le chirurgien et le Ped-PADSS. Quatre-vingt-quinze pour cent des patients avaient un Ped-PADSS≥9 après 2heures de surveillance.
Conclusion |
La majorité des enfants a rempli les critères de sortie au bout de 2heures de surveillance postopératoire. L’utilisation de ce score permettrait de réduire le temps d’hospitalisation à l’unité ambulatoire.

Background |
In adults, the Post-Anesthetic Discharge Scoring System (PADSS) was built to secure the discharge after outpatient surgery. We evaluate a pediatric adaptation: the Pediatric-PADSS (Ped-PADSS).

Prospective cohort.
Methods |
This was a prospective, observational, monocentric study for ambulatory patients. Ped-PADSS is built on 5 items each quoted 0, 1, or 2: hemodynamics, state of awakening, nausea/vomiting, pain and bleeding. A result ≥9/10 validated discharge if the anesthetist did not wish to review the patient, if the parents did not wish to revisit the anesthetist or if there was no hoarseness or dyspnea. The discharge was validated by the anesthetist and the surgeon. Ped-PADSS was made without the knowledge of the nursing team, one hour after return in service and repeated hourly. Addition of patient demographic data, the collection included the hours of leave by the anesthetist, surgeon and Ped-PADSS, the duration of hospital stay post procedure.
Results |
On 150 patients, 148 patients were allowed to go out with the Ped-PADSS, one patient was released despite a Ped-PADSS<9. One patient was hospitalized for a surgical bleeding in agreement with the anesthetist, surgeon and the Ped-PADSS. Ninety-five percent of patients had a Ped-PADSS ≥9 after 2hours monitoring in the ambulatory unit.
Conclusion |
The majority of the children have met the criteria for discharge at the end of 2hours postoperative monitoring. The use of this score could reduce the hospitalization time in ambulatory unit.
