L’angiosarcome épithélioïde (ASE) primitif de la surrénale est une tumeur maligne rare, pouvant être confondue avec d’autres néoplasies à l’examen histologique. Nous rapportons un cas chez un homme de 79 ans opéré d’une tumeur surrénalienne, découverte fortuitement à l’imagerie pour bilan d’un syndrome inflammatoire avec anémie. Le diagnostic histologique a été difficile en raison de l’aspect indifférencié et épithélioïde des cellules tumorales, dont la positivité pour les marqueurs immunohistochimiques épithéliaux orientait vers un carcinome métastatique avec lequel ce type d’angiosarcome est souvent confondu. Seule la recherche attentive d’une différenciation vasculaire, avec une positivité simultanée des marqueurs immunohistochimiques endothéliaux, a permis d’affirmer le diagnostic d’ASE.

The adrenal primary epithelioid angiosarcoma (ASE) is a rare malignant tumor which can be histologically confused with other neoplasms. We report one case in a 79-year-old man who underwent adrenal tumor surgery for a mass fortuitly discovered by imaging for examination of an inflammatory syndrome associated with anemia. The histological diagnosis was difficult because of the undifferentiated and epithelioid appearance of tumor cells, whose immunohistochemical epithelial markers positivity led to frequent confusion with a metastatic carcinoma. Careful research for vascular differentiation at histopathological study and expression of immunohistochimical endothelial markers, are crucial to confirm the diagnosis.
