La planification et la technique d'ostéotomie tibiale de valgisation par addition médiale utilisant un coin de phosphate tricalcique sont présentées. Le substitut, lentement résorbable, a une forme spécifique et des propriétés mécaniques permettant une ostéotomie stable par une courte incision et une ostéosynthèse simple par agrafes. Un contrôle radioscopique peropératoire rend fiable et reproductible cette technique. La rupture de la charnière latérale a été l'incident technique auquel expose cette technique imposant un complément de fixation par une agrafe latérale. La série comportait 58 genoux (55 patients), d'âge moyen 47 ans, porteurs de 43 arthroses fémoro-tibiales stade I d'Ahlback, 12 stades II et 3 stades III. La tolérance de l'implant a été constante et la consolidation obtenue la majeure partie des cas sans perte angulaire. Les complications ont été : 4 ruptures de la charnière latérale dont une a abouti à une pseudarthrose, et une infection à bas bruit. À moyen terme, la dégradation de l'implant a été très importante : parmi les 22 patients dont le recul était supérieur à 5 ans, l'implant était à peine visible 18 fois. Cette ostéotomie permet d'espérer une plus grande simplicité en cas de reprise. L'appellation dite « mini invasive » de cette technique est justifiée par la taille de l'incision et de l'ostéosynthèse et le rétablissement du capital osseux par l'effet ostéo-conducteur du substitut.

Purpose of the study |
We present a minimally invasive technique for tibial valgus osteotomy using a medial wedge composed of tricalcium phosphate.
Material and methods |
The bone substitute is composed of slowly resorbable tricalcium phosphate material shaped to the desired form and having mechanical properties allowing stable osteotomy via a short incision and staple fixation. Intraoperative fluoroscopy enables a reliable and reproducible technique. A lateral fixation staple is required because there is a risk the lateral hinge could break. This technique was used for 58 knees in 55 patients (mean age 47 years). According to the Ahlback classification of femorotibial degeneration, there were 43 grade I knees, 12 grade II, and 3 grade III.
Results |
The implant was well tolerated in all cases. Bone healing was achieved in most cases without loss of the osteotomy angle. Complications were: rupture of the lateral hinge in four cases leading to nonunion in one, one low-grade infection. Implant resorption at mid term was significant: among the 22 patients with a follow-up of more than 5 years, the implant was barely visible in 18.
Discussion |
This technique provides an easy way to achieve tibial valgus osteotomy without compromising future intervention. The technique can be considered to be minimally invasive because of the size of the incision, the minimal fixation required, and the bone sparing effect of the bone substitute.
