Encéphalopathie corticosensible associée à une thyroidite auto-immune : données actuelles - 01/04/08

Since the first description of encephalopathy associated with Hashimoto's thyroiditis in 1966, more than 100 cases of this entity called Hashimoto's encephalopathy or SREAT [steroid-responsive encephalopathy associated with auto-immune thyroiditis] have been reported. The two cases reported here illustrate different diagnostic criteria and offer the opportunity to discuss the many pathophysiological hypotheses based on particularly pertinent biological data, especially the activity of anti-TPO and anti-TG antibodies present in serum and cerebrospinal fluid and the recent discovery of anti--enolase antibody. Different treatment options are also discussed, in particular corticosteroid therapy.

Depuis la première description, en 1966, d'une encéphalopathie associée à une thyroïdite de Hashimoto, près d'une centaine d'observations de cette entité dénommée encéphalite de Hashimoto ou SREAT [steroid-responsive encephalopathy associated with auto-immune thyroïditis] ont été rapportées. Deux nouvelles observations permettent de rappeler les différents critères diagnostiques et de discuter les nombreuses hypothèses physiopathologiques. Le rôle des anticorps antithyropéroxydase et antithyroglobuline sériques et intrathécaux et la découverte récente d'un anticorps anti--énolase sont les éléments biologiques les plus pertinents. Les différentes orientations thérapeutiques, en particulier la corticothérapie, sont enfin abordées.
