Abstract |
Purpose |
Chemoradiotherapy (CRT) is considered as a standard treatment for unresectable and inoperable esophageal cancer (EC) patients. However, no consensus has been reached regarding the optimal synchronous chemotherapy regimen and the best combination of radiotherapy and chemotherapy. The aim of this study was to evaluate the efficacy and toxicity of raltitrexed plus cisplatin and docetaxel plus cisplatin to find a safe and effective concurrent chemotherapy schedule.
Patients and methods |
Our retrospective study included 151 EC patients treated with raltitrexed and cisplatin (RP) (n=90) or docetaxel and cisplatin (DP) (n=61) from 2011 till 2018. Survival outcomes and treatment related toxicity were analyzed between the two groups.
Results |
PFS and OS were 18 and 34 months in the RP group, while 13 and 20 months in the DP group (P=0.118 and P=0.270). The 1-, 2-, 3-year survival rates of the RP group were 71.1, 55.4 and 46.4%. For the DP group, these were 63.9, 44.3 and 37.6%, respectively. Compared with DP group, RP group received a superior CR rate (68.9% versus 52.5%, P=0.041). There was a trend that the total number of toxic reactions in RP group was lower than that in DP group (P=0.058).
Conclusions |
Even RP and DP groups have the similar survival outcomes and toxicity, raltitrexed/cisplatin get a higher complete response rate. Our study suggests that raltitrexed combined with cisplatin is a safe and effective concurrent chemotherapy regimen and it might be used as an alternative for cisplatin/5-FU and cisplatin/docetaxel in CCRT for EC patients.

La chimioradiothérapie est considérée comme un traitement standard pour les patients atteints d’un cancer de l’œsophage non résécable et inopérable. Toutefois, il n’y a pas de consensus sur le schéma optimal de chimiothérapie synchrone ni sur la meilleure combinaison de radiothérapie et de chimiothérapie. Cette étude avait pour objectif l’évaluation de l’efficacité et de la toxicité de l’association raltitrexed-cisplatine et celle docétaxel-cisplatine afin de trouver un schéma de chimiothérapie concomitante sûr et efficace.
Patients et méthodes |
Notre étude rétrospective a inclus 151 patients atteints du cancer de l’œsophage traités avec du raltitrexed et du cisplatine (RP) (n=90) ou du docétaxel et du cisplatine (DP) (n=61) de 2011 à 2018. La survie et la toxicité liée au traitement ont été analysées entre les deux groupes.
Résultats |
La survie sans progression et la survie globale étaient de 18 et 34 mois dans le groupe RP, contre 13 et 20 mois dans le groupe DP (p=0,118 et p=0,270). Les taux de survie du groupe RP sur des périodes de 1, 2 et 3 ans étaient de 71,1, 55,4 et 46,4 %. Pour le groupe DP, ils étaient respectivement de 63,9, 44,3 et 37,6 %. Par rapport au groupe DP, le groupe RP a bénéficié d’un taux de RC supérieur (68,9 % contre 52,5 %, p=0,041). Il y avait une tendance à un moindre nombre total de réactions toxiques dans le groupe RP par rapport au groupe DP (p=0,058).
Conclusions |
Les résultats de survie et la toxicité des groupes RP et DP sont similaires: l’association raltitrexed/cisplatine présente un taux de réponse complète plus élevé. Les résultats de notre étude démontrent que l’association raltitrexed-cisplatine est un schéma de chimiothérapie concomitante sûr et efficace et qu’il pourrait être utilisé comme alternative au cisplatine/5-fluoro-uracile et au cisplatine/docetaxel dans les cas de chimioradiothérapie concomitante chez les patients atteints du cancer de l’œsophage.
