Summary |
Objectives |
Lung lobectomy or segmentectomy is the first-line treatment for stage 1 or 2 non-small cell lung carcinoma (NSCLC) or selected metastasectomy. The development of thoracoscopy represents a major advancement in minimally invasive surgery but requires the use of highly specialised medical devices. The goal of this study was to provide an objective cost analysis for a surgical thoracic department to identify the most cost-effective surgical practices and to highlight actions for potential cost adjustments.
Methods |
Retrospective analysis of all consecutive patients undergoing lung resections for cancer in the first half of 2019. All costs were based on a micro-costing method or calculated following standard analytical principles, after which the final reimbursement was calculated, and the relative financial outcomes were determined.
Results |
A total of 162 patients were included in this study, of which 67 underwent thoracotomy and 95 underwent thoracoscopy. The overall cost of patient care was 2,135,547.80€, with a final reimbursement of 1,927,498.06€, for an overall loss of 208,049.74€. The cost differential was significantly worse for patients with hospitalisation severity levels 1 and 2 (P<0.001) as well as for thoracoscopy patients who need additional rehabilitation at the time of discharge (P=0.04). Actions will be necessary to improve the economic equilibrium associated with these treatments.
Conclusion |
Treatment of major pulmonary resections appears to be a net-financial loss for medical institutions. The evaluation of surgical practices with respect to the overall financial implications is an important step to ensure adequate compensation for patient care.

Objectif |
La résection pulmonaire est en première ligne pour le traitement de carcinome pulmonaire localisé ou de métastasectomies sélectionnées. Le développement de la voie mini-invasive via la thoracoscopie requiert des dispositifs médicaux coûteux. Le but de cette étude est une analyse des coûts de procédure pour mettre en avant de possibles ajustements.
Méthode |
Analyse rétrospective de toutes les résections pulmonaires réalisées pendant le premier semestre 2019. Les coûts et la valorisation des séjours sont calculés pour déterminer la balance financière.
Résultats |
162 patients ont été inclus, parmi lesquels 67 thoracotomies et 95 thoracoscopies. Le coût total de prise en charge était de 2 135 547,80€, la valorisation de 1 927 498,06€, soit une perte nette de 208 049,74€. La balance financière est mauvaise surtout pour les séjours à niveau de sévérité 1 et 2 (p<0,001) et les thoracoscopies qui demandent une réhabilitation postopératoire (p=0,04). Les actions d’amélioration sont discutées.
Conclusion |
Les résections pulmonaires majeures représentent un déficit net pour l’institution hospitalière. L’évaluation des pratiques chirurgicales réalisées est une étape importante pour une valorisation adéquate de la prise en charge.
