Improved proprioception in patients with a knee proprioceptive deficiency after wearing an elastic brace: A randomized crossover-controlled study - 05/05/26

Summary

Objectives

The purpose of this study was to provide new knowledge regarding proprioception and the benefit of wearing a proprioceptive knee brace in patients with a knee proprioceptive deficiency.

Methods

This is a prospective, controlled, randomized clinical investigation with a crossover design, in 15 patients with proprioceptive knee deficiency. Patients were asked to walk on a treadmill for 8 minutes with and without an elastic brace in a randomized order. The primary endpoint was change in functional proprioceptive capacity of the knee measured via the 3-axis composite score of the Y Balance Test, following brace wearing. Secondary endpoints compared the two conditions (with and without the brace) on skin sensitivity (baresthesia) using Semmes-Weinstein monofilaments, sensitivity to joint position (statesthesia) via the Joint Position Sense test, patient subjective evaluation via Visual Analog Scales on stability (static and dynamic), discomfort related to proprioceptive deficiency, motion fluidity and knee range of motion, as well as satisfaction and safety.

Results

Dynamic stability, characterized with the Y Balance Test, was significantly improved immediately after wearing the knee brace, compared with the condition without the brace ( P < 0.001). Skin sensitivity (baresthesia) and perception of knee positioning in space (statesthesia) were also significantly improved when wearing the brace, with an 18% increase in sensitivity threshold, and a reduction in leg extension angular error of 53.3% respectively ( P < 0.001). Patients’ perception of static and dynamic stability, as well as discomfort related to proprioception deficiency, were significantly improved when wearing the brace. No significant difference was observed for motion fluidity and range of motion.

Conclusion

Wearing the GenuAction knee brace significantly improved immediate knee proprioception in patients with proprioceptive deficiency, compared with the no-knee brace condition.

Résumé

Objectifs

Le but de cette étude était d’apporter de nouvelles connaissances concernant la proprioception et l’intérêt du port d’une genouillère proprioceptive chez les patients présentant un déficit proprioceptif du genou.

Méthodes

Il s’agit d’une étude clinique prospective, contrôlée et randomisée en cross-over , chez 15 patients présentant un déficit proprioceptif au niveau du genou. Les patients ont été invités à marcher sur un tapis roulant pendant 8 minutes avec et sans orthèse élastique dans un ordre aléatoire. Le critère d’évaluation principal était la modification de la capacité proprioceptive fonctionnelle du genou, mesurée via le score composite à 3 axes du Y Balance Test, après le port de la genouillère. Les critères d’évaluation secondaires comparaient les deux conditions (avec et sans genouillère) sur la sensibilité tactile (baresthésie) à l’aide des monofilaments de Semmes-Weinstein, la sensibilité à la position articulaire (statesthésie) via le test Joint Position Sense, le retour des patients via des échelles visuelles analogiques sur la stabilité (statique et dynamique), l’inconfort lié au déficit proprioceptif, la fluidité de mouvement du genou et l’amplitude des mouvements, ainsi que la satisfaction et la tolérance.

Résultats

La capacité proprioceptive fonctionnelle, caractérisée par le Y Balance Test, était significativement améliorée immédiatement après le port de la genouillère, par rapport à la condition sans orthèse ( p < 0,001). La sensibilité tactile (baresthésie) et la perception du positionnement du genou dans l’espace (statesthésie) ont également été significativement améliorées lors du port de la genouillère, avec une augmentation de 18 % du seuil de sensibilité et une réduction de l’erreur angulaire d’extension de la jambe de 53,3 % respectivement ( p < 0,001). La perception par les patients de la stabilité statique et dynamique ainsi que de l’inconfort lié au déficit proprioceptif se sont considérablement améliorés lors du port de la genouillère. Aucune différence significative n’a été observée concernant la fluidité et l’amplitude des mouvements.

Conclusion

Le port de la genouillère GenuAction a immédiatement amélioré de manière significative la proprioception du genou chez les patients présentant un déficit proprioceptif, par rapport à une condition en absence de genouillère.
