Cathétérisme artériel pulmonaire en réanimation : à quelle heure ? - 01/01/01

Resumen |
Objectifs : Répertorier les horaires de décision d'insertion d'un cathéter artériel pulmonaire (CAP) dans une unité de réanimation.
Type d'étude : Descriptive et prospective.
Patients : Patients d'une unité de réanimation chirurgicale de centre hospitalo-universitaire.
Méthodes : Les heures de décision d'insertion non programmée d'un CAP ont été répertoriées par tranches horaires d'une heure. À chaque CAP, les complications immédiates (ponction artérielle, pneumothorax, troubles du rythme ventriculaire, difficulté et/ou échec de cathétérisme de l'artère pulmonaire) ont été notées.
Résultats : Cent quarante-neuf patients (99 hommes, âge = 63 ± 15 ans, index de masse CORPORELLE = 25 ± 6 kg·m-2, score Apache II médian = 16) ont été inclus dans l'étude. Cent soixante-cinq décisions d'insertion de CAP ont été prises (16 patients ont nécessité deux CAP insérés à des sites différents). Neuf ponctions artérielles, deux pneumothorax, 32 cathétérismes difficiles, 42 troubles du rythme ventriculaire et huit échecs de cathétérisme de l'artère pulmonaire ont été répertoriés. L'insertion de CAP a été décidée 34 fois entre neuf et dix heures lors de la réunion médicale quotidienne. Pendant la nuit, cette fréquence était inférieure à 3 par heure entre une et neuf heures.
Conclusions : Les décisions d'insertion de CAP sont variables en fonction de l'horaire de la journée avec une baisse de fréquence pendant la nuit entre trois et neuf heures.

Objectives: Recording the time at which the insertion of a pulmonary artery catheter was decided.
Study design: Prospective and descriptive study.
Patients: Critically ill patients in an university hospital.
Methods: The times at which the insertion of a PAC was decided were recorded. For each pulmonary artery catheterization, immediate complications were recorded (arterial puncture, pneumothorax, ventricular arrhythmia, hard and failed pulmonary artery catheterization)
Results: One hundred and forty-nine patients were included (99 males, AGE = 63 ± 15 year, body mass INDEX = 25 ± 6 kg·m-2, median Apache II SCORE = 16). One hundred and sixty-five PAC insertions were decided (16 patients requiring two PACs). Nine arterial punctures, two pneumothoraces, 42 ventricular arrhythmias, 32 hard and eight failed pulmonary artery catheterizations occurred. Thirty-four PAC insertions were decided between 9 and 10 am WHEREAS = 3 PAC insertions per hour were decided between 1 and 9 am.
Conclusion: The rate of decision of PAC insertion are decreased during the second half of the night (1 to 9 AM).
