COMPLICATED ANORECTAL SEPSIS - 11/09/11

The exact incidence of anorectal sepsis remains unknown. One study stated that in a population of 300,000, there were 5000 to 6000 hospital admissions per annum due to anorectal sepsis.26 The incidence is three times higher in men than in women, 93, 133, 156, 184 a finding that has been attributed to occupation, hair distribution, increased sweating, and poor hygiene. The peak incidence is in the third and fourth decades of life.133, 156, 184 The majority of these patients have simple abscesses or fistulas, and therefore treatment is often straightforward. However, certain conditions predispose to the development of more complex anorectal sepsis, in which, as a result of the underlying disease or the nature of the infecting organism, the treatment is much more challenging and controversial. Specifically, this article addresses the diagnosis and treatment of anorectal sepsis associated with these challenging conditions, including hidradenitis suppurativa, hematologic disorders, necrotizing fasciitis, Crohn's disease, and AIDS.
