Acute otitis media (AOM), a purulent sudden infection of the middle ear, is one of the most frequent infections in children and the most common reason children are brought to physicians' offices.6 Almost one third of all visits to physicians for children younger than 12 months (almost 30 million per year in the United States) are for AOM.14, 20 Nearly two thirds of all children will have at least one episode of AOM, and nearly one third will have three or more episodes.7, 11 More than one quarter of all antibiotic prescriptions in the United States are written to treat AOM.7 Although it is diagnosed by primary care physicians several times daily and it has been investigated extensively, several basic controversies still exist concerning its treatment.
