Complications infectieuses après chirurgie carcinologique du pharyngo-larynx |
Objectif |
L'objectif de cette étude était d'analyser les complications infectieuses des interventions pour carcinome de l'hypopharynx et du larynx pour optimiser la prévention des risques septiques.
Matériels et méthodes |
Il s'agit d'une étude rétrospective de 608 exérèses totales ou partielles du pharyngo-larynx réalisée entre 1984 et 1999, ce qui représente 270 laryngectomies totales et 338 laryngectomies partielles. La méthode utilisée comprenait des mesures rigoureuses d'asepsie chirurgicale et une antibiothérapie protocolée, prolongée, et adaptée au type d'exérèse et aux antécédents de radiothérapie externe. Vingt facteurs ont été étudiés. Une analyse univariée, incluant 9 facteurs, et une analyse multivariée ont été réalisées.
Résultats |
Le taux global de complications infectieuses était de 11,1 %. Les pourcentages de fuites salivaires, d'infections du site opératoire et d'infections broncho-pulmonaires étaient respectivement de 11,9 % ; 1,5 % ; 1,5 % en cas d'exérèses totales et de 1,8 % ; 2,4 % ; 3,8 % en cas d'exérèses partielles. Les facteurs statistiquement significatifs étaient le stade tumoral, l'hématome postopératoire, la lymphorrhée postopératoire. Les infections étaient plus fréquentes dans les localisations tumorales pharyngées (14,19 %) que dans les localisations tumorales laryngées (9,23 %) sans différence statistiquement significative.
Conclusion |
Notre taux de complications infectieuses en chirurgie carcinologique du pharyngo-larynx, qui est faible après étude de la littérature, suggère l'intérêt des mesures d'asepsie chirurgicale et d'une antibiothérapie prolongée comme elle est recommandée dans la chirurgie dite contaminée.

Objective |
The objective of this study was to analyze the infectious complications of procedures for carcinoma of the hypopharynx and the larynx to optimize the prevention of septic risks.
Material and methods |
This retrospective study included 608 patients who underwent total (n=270) or partial (n=338) laryngectomy between 1984 and 1999. The procedures were performed under rigorous conditons of surgical asepsis and with prolonged antibiotic chemotherapy depending on the type of laryngectomy and past history of external radiotherapy. Twenty factors were studied. Univariate analysis, including 9 factors, and multivariate analysis were performed.
Results |
The global rate of infectious complications was 11.1%. The percentages of salivary leaks, other wound infections and non-wound infections were respectively 11.9%; 1.5%; 1.5% for total laryngectomy and 1.8%; 2.4%; 3.8% for partial laryngectomy. Statistically significant factors were tumor stage, postoperative hematoma, postoperative lymphorrhea and, to a lesser degree, pharyngeal localization.
Conclusion |
Our rate of infectious complications in oncologic pharyngeal and laryngeal surgery, which is low compared with data in the literature, emphasizes the importance of strict measures of surgical asepsis and prolonged antibiotic chemotherapy as is recommended for so-called contaminated surgery.
