La dacryo-cysto-rhinostomie est l'intervention chirurgicale qui supprime la gouttière lacrymale pour pouvoir marsupialiser le sac lacrymal dans la fosse nasale. Elle est indiquée principalement comme traitement des sténoses lacrymonasales symptomatiques (abcès, colique lacrymale, suppuration, épiphora invalidant) et accessoirement pour la mise en place d'une intubation bicanaliculaire lorsque le cathétérisme vertical sans fausse route n'est pas possible. La principale difficulté est de bien repérer et bien gérer la zone chirurgicale idéale au sein des multiples variantes anatomiques de la fosse nasale. Elle s'accommode mal des fosses nasales que l'on ne peut élargir. Ses résultats peuvent être comparés favorablement à ceux de la voie externe.

The dacryocystorhinostomy is the surgery that removes the lacrymal duct for the fistulization of the lacrymal sac into the nasal cavity. It is primarily indicated for the treatment of symptomatic lacrymo-nasal stenosis (abscess, lacrymal colic, discharge, disabling epiphora) and secondarily for the establishment of a bicanalicular intubation when the vertical catheter without false route is not possible. The main difficulty is to properly identify and manage the ideal surgical area in multiple anatomic variations of the nasal cavity. This area badly accommodates with nasal cavities which cannot be expanded. These results can be favorably compared with results obtained from the the external route.
