Introduction |
L'aspergillose est une infection qui peut se révéler sous des formes cliniques différentes selon l'état immunitaire du patient. Son diagnostic, qui peut être difficile dans certaines circonstances (immunodépressions en particulier), repose sur un faisceau d'arguments cliniques, radiologiques et biologiques. Le dépistage des anticorps spécifiques est un élément important du diagnostic des formes non invasives chez le sujet immunocompétent. Plusieurs techniques sérologiques sont couramment utilisées.
Objectifs |
Evaluer les performances de l'hémagglutination indirecte dans le dépistage des anticorps anti-aspergillaires et déterminer la place de cette technique dans le sérodiagnostic des aspergilloses.
Méthodes |
Les performances de l'hémagglutination indirecte ont été comparées à celles de l'électrosynérèse et de l'immunoélectrophorèse choisie comme référence. Pour ces trois techniques, les auteurs ont déterminé la sensibilité, la spécificité, le coût et la faisabilité.
Résultats |
Cent soixante-seize sérums ont été testés avec les trois techniques : 39 provenaient de patients ayant des aspergilloses de formes cliniques différentes, 137 provenaient de sujets contrôles. La sensibilité des tests a été de 32 % pour l'hémagglutination indirecte (seuil à 1/640), 64 % pour l'électrosynérèse et 92 % pour l'immunoélectrophorèse. La spécificité a été de 93 % pour l'hémagglutination indirecte, 100 % pour l'électrosynérèse et l'immunoélectrophorèse. En abaissant le seuil de positivité de l'hémagglutination indirecte à 1/160, la sensibilité de cette technique atteint 76 % et devient comparable à celle de l'électrosynérèse avec une spécificité de 88 %.
Conclusion |
L'hémagglutination indirecte étant plus facile et plus rapide à mettre en oeuvre, celle-ci pourrait être une alternative intéressante dans le dépistage des anticorps anti-aspergillaires à condition d'abaisser le seuil du fabricant à 1/160 et de confirmer par l'immunoélectrophorèse tout résultat positif en hémagglutination indirecte.
Evaluation of the indirect hemagglutination assay for antibodies detection in aspergillosis diagnosis |
Introduction |
Aspergillus infection may lead to various clinical patterns according to the immune status of the patients. The diagnosis of aspergillosis may be difficult to achieve in some situations, particularly in immunosuppressed patients, and usually results from the association of clinical, radiological and biological features. Detection of specific antibodies remains of great value in diagnosing aspergilloma or allergic bronchopulmonary aspergillosis in immunocompetent patients.
Objectives |
To evaluate the performances of indirect hemagglutination for Aspergillus antibody detection and to determine the place of this method in the serologic diagnosis of aspergillosis.
Methods |
We compared three commercially available assays for the detection of Aspergillus antibodies: hemagglutination, electrosyneresis and immunoelectrophoresis which was the reference test. The evaluation of the assays comprised sensitivity, specificity, cost and feasibility.
Results |
One hundred and seventy six sera were analyzed with the three assays: 39 were collected from patients with aspergillosis (different patterns), 137 from controls. Sensitivity was 32 % with the hemagglutination assay (threshold 1/640), 64 % with the electrosyneresis assay and 92 % with the immunoelectrophoresis assay. Specificity was 93 % with the hemagglutination assay, 100 % with the electrosyneresis and the immunoelectrophoresis assays. When the threshold of the hemagglutination assay was lowered at 1/160 the sensitivity and specificity became 76 % and 88 % respectively. The feasibility was better with the hemagglutination assay and it was less time-consuming than the other methods.
Conclusion |
These results suggest that the hemagglutination assay could be performed for the detection of Aspergillus antibodies if the threshold is lowered at 1/160 and the positive results are confirmed with the immunoelectrophoresis assay.
