Clinicians and parents often find themselves with difficult decision making problems related to extremely premature birth or fetal malformations. This article examines these issues, the role of parents and clinicians in the decision, and a process from decision making, with medical ethical, social, and legal perspectives. Case examples are used to highlight certain issues. The authors also present practical advice and conclusions for the clinician.
