Therapeutic management of hand burns. The significant role of physiotherapy and occupational therapy - 07/12/23

The increase in survival of patients with severe burns brings to the fore the challenge of rehabilitation of these patients. A hand burn injury has a significant impact on the individual's skin, functionality, everyday activities and quality of life. The burn requires the cooperation of a well-organized and specialized interdisciplinary team, which consists of plastic surgeons, nursing staff, physiotherapists, occupational therapists etc.
Physiotherapy and occupational therapy have a significant role in hand burn rehabilitation in improving hand function, range of motion, hand strength, scar outcome, preventing the development of contractures, decreasing edema and promoting psychological well-being and social reintegration of individuals.
The burn unit of our hospital conducted a study on upper extremity burns studying the time and quality of rehabilitation such as hand function, range of motion, muscle strengthening, activities of daily living performance depending on the treatment method followed such as surgical or conservative treatment.
Within 5 years, 297 patients with hand burns were treated, about 2/3 were men with thermal, chemical and electrical burns. Most patients were treated conservatively and several surgically. Surgical treatment resulted in increase of recovery time due to immobilization period. However, in the last six months, 5 patients underwent enzymatic cleaning, avoiding the immobilization time. The outcomes obtained from these patients included an increase in elasticity, relative reduction of scars and pigmentation, fewer functional disabilities, better aesthetics, fewer infections, reduction in hospitalization and costs.
We must not forget that time works in favor of our patient, so it is our duty to study and find anything (methods, materials) such as enzymatic cleaning, that will reduce recovery time and hospitalization time and try to reduce contusions, and deformities that will cause serious disabilities to the burn victim.
Physiotherapy and Occupational therapy have a significant role in hand burn rehabilitation. Immobilization time has a negative effect in rehabilitation. Enzymatic cleaning decreases the negative effect, facilitates the procedure and improves the outcome of our therapy.
