Nasopharyngeal cancers are rare in France but much more common in endemic regions such as Southeast Asia, North Africa, and Alaska. Radiotherapy remains the cornerstone of treatment, combined with chemotherapy for locally advanced cases. The technical aspects of radiotherapy are complex due to the proximity of numerous critical structures. Recent studies have explored the benefits of combining systemic therapies, such as immunotherapy, with radiotherapy, the advantage of de-escalating irradiated lymph node volumes to reduce toxicity while maintaining therapeutic efficacy, and the value of monitoring via Epstein-Barr virus circulating tumour DNA to improve patient follow-up. Here, we present the updated recommendations of the French society of radiation oncology (Société française de radiothérapie oncologique, SFRO) concerning the indications and technical modalities of radiotherapy in nasopharyngeal cancers.

Les cancers du nasopharynx sont rares en France, mais beaucoup plus fréquents dans d’autres régions où ils sont endémiques, notamment en Asie du Sud-Est, en Afrique du Nord et en Alaska. La radiothérapie demeure le traitement de référence, souvent associée à la chimiothérapie. Les modalités techniques de la radiothérapie sont complexes en raison de la proximité de nombreuses structures critiques. Des études récentes ont exploré les bénéfices des thérapies systémiques combinées à la radiothérapie, notamment l’immunothérapie, l’avantage de la désescalade des volumes ganglionnaires irradiés pour réduire la toxicité tout en maintenant l’efficacité thérapeutique, ainsi que l’intérêt du suivi via l’ADN tumoral circulant du virus d’Epstein-Barr pour améliorer la surveillance des patients. Nous présentons ici la mise à jour des recommandations de la Société française de radiothérapie oncologique (SFRO) concernant les indications et les modalités techniques de la radiothérapie dans les cancers du nasopharynx.
