Le kyste hydatique primitif rénal constitue une affection exceptionnelle et une localisation rare, même au Maghreb où l’hydatidose sévit à l’état endémique. L’échinococcose kystique est une zoonose parasitaire, due au stade larvaire d’ Echinococcus granulosus , touchant préférentiellement le foie. Les atteintes extra-hépatiques, en particulier rénales, sont rares et leur prise en charge est encore peu codifiée.

Nous rapportons le cas d’un jeune homme de 28 ans présentant une hématurie avec émission de vésicules hydatiques dans les urines. Le diagnostic a été confirmé par l’imagerie et l’identification parasitologique des protoscolex. Une communication du kyste avec les voies urinaires a été objectivée. Le patient a bénéficié d’un traitement chirurgical conservateur avec évolution favorable. Ce cas illustre l’importance du diagnostic parasitologique et de l’imagerie dans les formes compliquées.

Primary renal hydatid cyst is a rare condition and an uncommon location, even in the Maghreb where hydatidosis is endemic.

Cystic echinococcosis is a parasitic zoonosis caused by the larval stage of Echinococcus granulosus, primarily affecting the liver. Extra-hepatic involvement, particularly renal, is rare and its management remains poorly standardized. We report the case of a 28-year-old man presenting with hematuria and passage of hydatid vesicles in urine. Diagnosis was confirmed by imaging and parasitological identification of protoscolices. A fistula between the cyst and urinary tract was detected. The patient underwent conservative surgical treatment with favorable outcome. This case highlights the importance of parasitological and imaging tools in complicated forms.
