La granulomatose avec polyangéite (GPA) (ex-maladie de Wegener) est une vascularite systémique à ANCA (anticorps anti-cytoplasme des polynucléaires neutrophiles) avec atteinte pulmonaire fréquente. Nous rapportons le cas d’une patiente présentant une GPA de localisation pulmonaire isolée et de présentation clinique pseudotumorale. En présence des critères histopathologiques majeurs de la GPA (lésions de vascularite, nécrose parenchymateuse et inflammation granulomateuse), le diagnostic de GPA peut être proposé après avoir exclu les principaux diagnostics différentiels (infection, granulomatose sarcoïdosique nécrosante, granulomatose éosinophilique avec polyangéite et granulomatose lymphomatoïde). Cependant, ces critères sont souvent absents sur biopsie transthoracique, justifiant la réalisation d’une biopsie chirurgicale en cas de forte probabilité clinique. Le diagnostic final de GPA repose sur l’intégration des données cliniques, d’imagerie, d’anatomopathologie et de biologie (présence éventuelle d’ANCA).

Granulomatosis with polyangiitis (GPA) (formerly Wegener's disease) is a systemic ANCA (anti-neutrophil cytoplasmic antibody) vasculitis that frequently affects the lungs. We report the case of a patient with isolated pulmonary GPA and a pseudotumoral clinical presentation. In the presence of the major histopathological criteria for GPA (vasculitis lesions, parenchymal necrosis, and granulomatous inflammation), a diagnosis of GPA can be proposed after excluding the main differential diagnoses (infection, necrotizing sarcoidosis granulomatosis, eosinophilic granulomatosis with polyangiitis, and lymphomatoid granulomatosis). However, these criteria are often absent on transthoracic biopsy, justifying a surgical biopsy in cases of high clinical probability. The final diagnosis of GPA is based on the integration of clinical, imaging, anatomopathological, and biological data (possible presence of ANCA).
