À partir d’une lecture de l’essai de Susan Sontag, La maladie comme métaphore (1978), cet article interroge les modalités d’énonciation de la maladie et les effets que peut produire, jusque dans la relation de soin, l’opération de métaphorisation qui affecte la représentation de certaines pathologies. Nous montrons comment Sontag reconstitue les réseaux d’images dans lesquels se sont sédimentées les représentations sociales et culturelles dominantes de deux maladies particulières, la tuberculose et le cancer, afin de souligner à la fois leur parenté et leur dissociation. De telles représentations forment également le filtre à partir duquel la relation médicale aux patients trouve à s’élaborer. L’appel à une certaine hygiène métaphorique interroge ainsi la responsabilité énonciative propre des soignants dans la manière dont la maladie est dite, annoncée et accompagnée.

Based on a reading of Susan Sontag's essay Illness as metaphor (1978), this paper examines the modes of enunciation of illness and the effects that the operation of metaphorization, shaping the representation of certain pathologies, can produce, including within the therapeutic relationship. We show how Sontag reconstructs the networks of images in which dominant social and cultural representations of two particular diseases, tuberculosis and cancer, have become sedimented, in order to highlight both their affinities and their divergences. Such representations also constitute the filter through which the medical relationship with patients is built. The call for a metaphorical hygiene thus raises questions about the specific enunciative responsibility of healthcare professionals in how illness is articulated, disclosed, and accompanied.
