Current state of management of heart failure patients at Le Mans Hospital Center

L’insuffisance cardiaque (IC) est une pathologie chronique majeure associée à une morbi-mortalité importante. Malgré des recommandations actualisées de l’ESC 2021, la mise en pratique reste souvent insuffisante. L’objectif est de décrire les prescriptions à l’entrée et à la sortie des patients hospitalisés pour IC au centre hospitalier du Mans (CHM), et d’en évaluer l’adéquation aux recommandations. Étude descriptive monocentrique, menée de juin 2023 à juin 2024 incluant 290 patients selon leur FEVG : altérée, préservée (FEVGp) ou indéterminée (FEVGi). L’âge moyen était de 81,4 ans, à prédominance féminine pour les FEVGp et FEVGi. À l’admission, la prise en charge reposait surtout sur les diurétiques, tandis que les traitements de fond restaient sous-utilisés. À la sortie, chez les patients à FEVG altérée, 77,0 % recevaient une gliflozine, 66,7 % un inhibiteur du système rénine-angiotensine, 82,3 % un bêtabloquant et 63,5 % un diurétique épargneur de potassium ; la quadrithérapie concernait 39,6 % des patients. Chez les patients à FEVGp et FEVGi la monothérapie prédominait, et jusqu’à 30,0 % des patients FEVGi ressortaient sans traitement. L’hypertension artérielle et les troubles du rythme étaient les comorbidités les mieux prises en charge, contrairement à l’anémie et à la prévention vaccinale. Le suivi révélait un taux élevé de réhospitalisations (près de 50 %) et une mortalité à un an de 26,0 %. Cette étude souligne l’importance de l’hospitalisation pour optimiser la prise en charge de l’IC et les axes d’optimisations que le pharmacien clinicien pourrait avoir dans l’initiation, la titration des traitements, la gestion des comorbidités et la prévention vaccinale chez ces patients.

Heart failure (HF) is a major chronic condition associated with high morbidity and mortality. Despite the updated the European Society of Cardiology (ESC) 2021 guidelines, their implementation in clinical practice often remains suboptimal. The objective of this study was to describe drug prescriptions at admission and discharge for patients hospitalized with a HF at Le Mans Hospital (CHM), and to assess their compliance with current guidelines. This was a single-center descriptive study conducted from June 2023 to June 2024, including 290 patients according to their left ventricular ejection fraction (LVEF): impaired, preserved (pLVEF), or indeterminate (iLVEF). The average age was 81.4 years, with a female predominance in the preserved and indeterminate LVEF groups. At admission, treatment was based mainly on diuretics, while background treatments remained underused. At discharge, among patients with impaired LVEF, 77% were receiving a gliflozin, 66.7% a renin-angiotensin system inhibitor, 82.3% a beta-blocker, and 63.5% a potassium-sparing diuretic; quadruple therapy was used in 39.6% of patients. In patients with pLVEF and iLVEF, monotherapy predominated, and up to 30.0% of iLVEF patients were discharged without treatment. Hypertension and arrhythmias were the most commonly managed comorbidities, unlike anemia and vaccination. Follow-up revealed a high rate of rehospitalization (nearly 50%) and a one-year mortality rate of 26%. This study highlights the importance of hospitalization in optimizing the management of HF and identifies areas where clinical pharmacist could play a key role in initiatiating and titrating treatments, managing comorbidities, and ensuring vaccination coverage for these patients.
