Après un bref rappel sur l’histoire de la psychiatrie, la question du diagnostic des pathologies mentales est précisée et articulée à la continuité des soins, complément incontournable d’une psychiatrie publique digne de ce nom. Une réflexion sur la relation transférentielle spécifique aux pathologies archaïques (autisme, psychose, schizophrénie, pathologies-limites) est ensuite abordée en lien avec le diagnostic et la prise charge dans la durée. La déstructuration du service public de psychiatrie et la fétichisation du diagnostic au détriment de la prise en charge risquent de nuire gravement à la continuité des soins et d’introduire une rupture éthique dans la fonction soignante qui portera préjudice aux patients.

After a brief historical overview of psychiatry, the issue of diagnosis in psychiatry is clarified and linked to continuity of care, an essential complement to any public psychiatric service worthy of the name. Reflections on the transferential relationship specific to archaic illnesses (autism, psychosis, schizophrenia, borderline pathologies) are then addressed in relation with diagnosis and long-term care. The destructuring of the public psychiatric service and the fetishization of diagnosis to the detriment of support risk seriously harming the continuity of care and introducing an ethical break in the nursing function that will harm patients.
