Les troubles oculomoteurs sont fréquents chez les patients atteints de paralysie cérébrale et persistent à l’âge adulte, avec un retentissement significatif sur les activités quotidiennes et la qualité de vie. Pourtant, leur prise en charge reste encore peu décrite dans cette population. Cet article a pour objectif d’analyser l’intérêt d’une approche orthoptique individualisée chez l’adulte, en s’appuyant à la fois sur des rappels théoriques et sur l’étude d’un cas clinique. Après un rappel des principaux troubles oculomoteurs rencontrés, nous présentons le bilan orthoptique initial réalisé dans l’enfance, puis le suivi au long cours d’un patient adulte atteint de paralysie cérébrale. Le témoignage du patient apportera un éclairage complémentaire sur l’impact des troubles visuels et sur l’expérience vécue de la rééducation.

Oculomotor disorders are common in patients with cerebral palsy and persist into adulthood, with a significant impact on daily activities and quality of life. However, their management is still poorly described in this population. The aim of this article is to analyze the interest of an individualized orthoptic approach in adults, based on both theoretical data and the study of a clinical case. After a review of the main oculomotor disorders encountered (chasing, saccades, fixation or strabismus), as well as their functional consequences, we present the initial orthoptic assessment of an adult patient with cerebral palsy. The patient's testimony will shed additional light on the impact of visual disorders and on the lived experience of rehabilitation.
