Introduction |
La polymédication est présente chez 72 % des patients de plus de 75 ans en France et l’hyperpolymédication 25 %. Si elle peut être justifiée par la polypathologie, la polymédication expose à un risque accru de iatrogénie et nécessite donc d’être réévaluée régulièrement. En Normandie, le taux de patients avec au moins un médicament potentiellement inapproprié (MPI) est de 40 %. Le parcours OPTIMEDOC, déployé en Normandie et au centre hospitalier universitaire de Caen depuis 2022, a pour finalité l’optimisation des prescriptions médicamenteuses des patients âgés, via des RCP impliquant professionnels hospitaliers (pharmaciens cliniciens, gériatres) et de ville (médecin traitant).
Matériels et méthodes |
Une mise en perspective des résultats du parcours OPTIMEDOC a été réalisée avec les données publiées dans la littérature des parcours d’optimisation thérapeutique similaires, IATROPREV, OPERAM et SENATOR. Cette comparaison portait sur les caractéristiques organisationnelles des parcours, les populations incluses et les taux de mise en œuvre des recommandations. Les facteurs associés au succès du parcours OPTIMEDOC au CHU de Caen ont été identifiés à partir d’une analyse qualitative des éléments organisationnels, institutionnels et professionnels ayant contribué à son déploiement et à son efficacité.
Résultats et discussion |
Un an après leurs préconisations, 69 % des recommandations étaient toujours suivies, dont 82 % pour les déprescriptions. Ces résultats sont supérieurs aux parcours européens OPERAM (50 %) et SENATOR (15 %) qui n’impliquent pas activement les professionnels de ville et au sein desquels le gériatre ne rédige pas de nouvelle ordonnance. Ils sont par contre inférieurs au parcours IATROPREV (83 %) qui implique activement le pharmacien d’officine en RCP et bénéficie d’un coordinateur de parcours. Les principaux facteurs de succès d’OPTIMEDOC au CHU de Caen reposent sur l’implication active des gériatres et pharmaciens cliniciens, l’adhésion des professionnels de ville, un ancrage institutionnel régional structuré avec ressources dédiées, et l’utilisation d’un outil numérique facilitant le suivi et la traçabilité des optimisations.
Conclusion |
Les parcours OPTIMEDOC et IATROPREV sont des parcours efficaces car interventionnistes et coordonnés avec la ville, cependant leur efficacité sur des critères cliniques reste à évaluer.

Introduction |
In France, polypharmacy and hyperpolypharmacy affects 72 and 25% of adults aged over 75 respectively. Polypharmacy is often driven by multimorbidity. It increases the risk of adverse drug reactions, and therefore requires regular medication reviews. In Normandy, 40% of adults use at least one potentially inappropriate medication (PIM). The OPTIMEDOC program, implemented in Normandy and at the Caen University Hospital since 2022, aims to optimize older patients therapeutics through multidisciplinary medication reviews, involving hospital teams (clinical pharmacist, geriatrician) and community healthcare professionals (general practitioners).
Materials and methods |
The outcomes of the OPTIMEDOC program were contextualised and compared with published data from similar medication optimization programs, IATROPREV, OPERAM, and SENATOR. This comparison focused on organisational aspects, patient populations, and rates of recommendations’ implementation. Factors associated with the success of the OPTIMEDOC program at Caen University Hospital were identified through a qualitative analysis of organisational, institutional, and professional elements contributing to its implementation and effectiveness.
Results and discussion |
One year on, 69% of total recommended optimizations had been implemented with 82% for deprescriptions. While these results are higher than those of the European programs OPERAM (50%) and SENATOR (15%), in which community-based professionals are not actively involved and geriatricians do not issue new prescriptions, they are lower than those of IATROPREV (83%), which involves community pharmacists in the multidisciplinary meetings and benefits from a dedicated care-pathway coordinator. The main factors associated with the success of OPTIMEDOC at Caen University Hospital include the active involvement of geriatricians and clinical pharmacists, the engagement of community healthcare professionals, a structured regional institutional framework with dedicated resources, and the use of a software facilitating the follow-up and the traceability of medication optimization.
Conclusion |
The OPTIMEDOC and IATROPREV programs are effective because they are interventional and well-coordinated with community healthcare professionals, however, their effectiveness on clinical outcomes remains to be evaluated.
