Glaucoma is the leading cause of irreversible blindness worldwide. High myopia, an independent risk factor for this disease, adds complexity to its diagnosis, follow-up, and medical and surgical management. These guidelines represent a consensus of French glaucoma and retina experts on the management of glaucoma in high myopia.
The therapeutic strategy and follow up modalities are similar to those used for primary open angle glaucoma, the goal being to achieve a target intra-ocular pressure (IOP) that prevents the onset or rapid worsening of glaucomatous damage.
In case of elevated IOP and diagnostic uncertainty due to extensive chorioretinal alterations interfering with the fundus assessment and interpretation of tests, IOP lowering treatment may be initiated to prevent the risk of irreversible glaucomatous damage in addition to that already associated with high myopia.
The increased incidence of postoperative complications should be taken into account when considering a surgical management.

Le glaucome est la première cause de cécité irréversible dans le monde. La forte myopie, facteur de risque indépendant de cette maladie, complexifie son diagnostic, son suivi ainsi que sa prise en charge médicale et chirurgicale. Ces recommandations représentent un consensus d'experts français du glaucome et de la rétine sur la prise en charge du glaucome associé à une forte myopie.
La stratégie thérapeutique et les modalités de suivi sont similaires à celles utilisées pour le glaucome primitif à angle ouvert, l'objectif étant d'atteindre une pression intra-oculaire (PIO) cible permettant de prévenir la survenue ou l'aggravation rapide de l'atteinte glaucomateuse.
En cas de PIO élevée et d'incertitude diagnostique liée à des altérations chorio-rétiniennes étendues gênant l'examen du fond d'œil et l'interprétation des examens complémentaires, un traitement hypotonisant pourra être instauré afin de prévenir le risque d'atteinte glaucomateuse irréversible, qui s'ajoute à celui déjà associé à la forte myopie.
L'augmentation de l'incidence des complications post-opératoires doit être prise en compte lorsqu’une prise en charge chirurgicale est envisagée.
