Introduction |
L’arrêté du 21 février 2023 a instauré le renouvellement et l’adaptation des prescriptions (RAP) par le pharmacien hospitalier, afin de renforcer la continuité et la sécurité des traitements.
Matériel et méthodes |
Cette étude prospective descriptive avait pour objectif d’évaluer la mise en œuvre du RAP en oncologie médicale au CHU de Nantes. L’étude a été conduite sur 50 jours entre juin et août 2025 auprès de patients hospitalisés bénéficiant d’une activité de pharmacie clinique. Les données recueillies portaient sur la nature et le volume des RAP ainsi que le suivi à j3. Des entretiens individuels semi-directifs avec les oncologues ont été menés pour explorer le ressenti et l’expérience du dispositif.
Résultats |
Au total, 334 RAP ont été réalisées chez 57 patients : 290 renouvellements directs (86,8 %), 33 adaptations directes (9,9 %) et 11 adaptations concertées (3,3 %). À j3, 98 % des renouvellements et 91 % des adaptations étaient maintenues. Les oncologues ont jugé le dispositif sécurisant tout en soulignant l’importance d’une traçabilité claire et d’une concertation interprofessionnelle.
Discussion |
Ces données suggèrent que le RAP peut jouer un rôle clé dans la sécurisation et l’optimisation de la prise en charge médicamenteuse des patients en oncologie.

Introduction |
The decree of 21 February 2023 introduced prescription renewal and adaptation (RAP) by hospital pharmacists to improve the continuity and safety. The aim of this prospective descriptive study was to evaluate the implementation of RAP in medical oncology at Nantes University Hospital.
Materials and method |
The study was conducted over 50 days period between June and August 2025 among hospitalised patients receiving clinical pharmacy services. The data collected focused on the type and volume of RAP and their follow-up on day 3. Semi-structured individual interviews with oncologists were conducted to explore their perceptions and experiences of the system.
Results |
A total of 334 RAP were carried out in 57 patients: 290 direct renewals (86.8%), 33 direct adaptations (9.9%) and 11 concerted adaptations (3.3%). On day 3, 98% of renewals and 91% of adaptations were maintained. Oncologists considered the system to be safe, while emphasising the importance of clear traceability and interprofessional collaboration.
Discussion |
These findings suggest that RAP can play a key role in ensuring the safety and optimisation of medication management in oncology patients.
