Radical gastrectomy with D2 lymphadenectomy remains the cornerstone of curative treatment for gastric adenocarcinoma. Multiple randomized trials have demonstrated the oncological safety of minimally invasive gastrectomy while offering advantages in postoperative recovery and reduced surgical trauma. The Hugo™ RAS system differs from conventional robotic systems because of its modular architecture consisting of independent arm carts. While this design provides greater flexibility in operating room configuration, it introduces unique challenges regarding port placement, docking geometry and prevention of arm collisions. Currently, there is a paucity of literature describing robotic radical gastrectomy using modular robotic platforms.We present our standardized approach of robotic radical gastrectomy using the Hugo™ RAS system with particular emphasis on port placement, docking strategy and technical aspects of D2 lymphadenectomy.
