La vision est un processus fondamentalement actif dont l’oculomotricité constitue le premier maillon, guidant la préhension, structurant la posture et conditionnant la marche. Dans la paralysie cérébrale (PC), cette synergie visuomotrice est rompue. Les déficits oculomoteurs, présents chez jusqu’à 98 % des enfants tétraplégiques, compromettent la coordination du geste et fragmentent la perception du mouvement, tandis que l’instabilité posturale contraint le système visuel à une fonction d’équilibration, réduisant sa disponibilité pour l’exploration et les apprentissages. Un cercle dysfonctionnel s’installe alors, dans lequel déficits moteurs, visuels et cognitifs se potentialisent mutuellement. En s’appuyant sur la littérature récente, cet article analyse ces interactions pathologiques et défend une approche rééducative décloisonnée, dans laquelle l’orthoptiste occupe une place centrale pour restaurer le lien fonctionnel entre perception visuelle et action, et permettre ainsi à l’enfant de s’engager activement dans son développement.

Vision is a fundamentally active process in which oculomotricity constitutes the first building block, guiding prehension, structuring posture and underpinning gait. In cerebral palsy (CP), this visuomotor synergy is disrupted. Oculomotor deficits, present in up to 98 % of children with tetraplegia, compromise movement coordination and fragment motion perception, while postural instability constrains gaze to a balance regulation function, reducing its availability for exploration and learning. A dysfunctional cycle then develops, in which motor, visual and cognitive deficits mutually reinforce one another. Drawing on recent literature, this article analyses these pathological interactions and advocates for an integrated rehabilitative approach, in which the orthoptist plays a central role in restoring the functional link between visual perception and action, thereby enabling the child to engage actively in their own development.
