Les récents progrès biomédicaux ont profondément transformé la médecine, ouvrant la voie à une approche prédictive, personnalisée et technologique du soin. Pourtant, ces avancées contrastent avec la faiblesse persistante des politiques de prévention et de santé publique, alors même que les déterminants sociaux, environnementaux et alimentaires jouent un rôle majeur dans la santé des populations. Face au vieillissement démographique, à l’augmentation des maladies chroniques et aux tensions sur les ressources, une réflexion prospective s’impose sur l’organisation du système de santé qui ne doit plus être uniquement un système de soins et sur les choix collectifs qui le structurent. L’article interroge les nouveaux enjeux bioéthiques à l’interface entre science et société, en insistant sur la nécessité de restaurer la confiance, de renforcer la démocratie sanitaire et de promouvoir une éthique de la sobriété dans le soin. Il explore enfin l’évolution du métier de médecin, appelé à conjuguer innovation technologique, coordination des parcours et exigence humaniste, afin de refonder un pacte social durable autour de la « bonne santé ».

Recent advances in biomedicine have profoundly transformed medical practice, fostering a predictive, personalised and technology-driven approach to care. However, these innovations stand in sharp contrast with the persistent underinvestment in public health and prevention, despite the overwhelming influence of social and environmental determinants on population health. In the context of demographic ageing, the rise of chronic diseases and increasing pressure on healthcare resources, a forward-looking reflection on healthcare organization and collective priorities is urgently needed. This article examines the emerging bioethical challenges at the interface of science and society, emphasizing the need to rebuild trust, strengthen health democracy and promote an ethic of sobriety in care. It also explores the evolving role of physicians, who must combine technological expertise, care coordination and humanistic values in order to renew the social contract centred on healthcare.
