Ce cas clinique décrit la prise en charge anesthésique et analgésique d’une chienne stérilisée Retriever du Labrador âgée de huit ans, présentée pour l’exérèse d’un mastocytome cutané cervical. L’anesthésie a été conduite sans opioïdes, en s’appuyant sur une stratégie multimodale intégrant un bloc interfascial du plexus cervical, une perfusion continue de kétamine et l’administration d’un anti-inflammatoire non stéroïdien. Bien établi en médecine humaine pour les interventions chirurgicales en région cervicale, ce bloc reste très peu utilisé en médecine vétérinaire, avec seulement deux descriptions publiées à ce jour chez le chien. Le bloc a été réalisé sous guidage échographique et aucune complication per- ou postopératoire n’a été constatée. Le monitoring peropératoire n’a révélé aucun signe de nociception et aucune analgésie de secours n’a été nécessaire. Le réveil s’est déroulé calmement et la gestion analgésique durant la période postopératoire immédiate était satisfaisante, permettant une sortie d’hospitalisation le jour même. Ce cas illustre l’efficacité d’une prise en charge analgésique sans opioïdes reposant sur une technique d’anesthésie locorégionale innovante : le bloc du plexus cervical.

This clinical case describes the anesthetic and analgesic management of an eight-year-old spayed female Labrador Retriever presented for the excision of a cervical cutaneous mastocytoma. Anesthesia was conducted without opioids, relying on a multimodal strategy integrating an interfascial cervical plexus block, a continuous ketamine infusion, and the administration of a non-steroidal anti-inflammatory drug. Well established in human medicine for cervical surgical procedures, this block remains very rarely used in veterinary medicine, with only two published descriptions to date in dogs. The block was performed under ultrasound guidance, and no peri- or postoperative complications were observed. Perioperative monitoring revealed no signs of nociception, and no rescue analgesia was required. Recovery proceeded smoothly, and analgesic management during the immediate postoperative period was satisfactory, allowing discharge on the same day. This case illustrates the efficacy of opioid-free analgesic management based on an innovative locoregional anesthesia technique: the cervical plexus block.
